摘  要:为探讨晶体溶解性青光眼的临床特点及治疗方法,以期引起对该病的广泛认识,促进早期正确诊治,减少病人痛苦,对7例晶体溶解性青光眼患者的临床资料进行分析。结果:7例患者均系老年人,均有白内障病史,无青光眼病史。起病突然,眼压升高,视力急骤下降,角膜混浊水肿,前房较深或正常,房水中有灰白色或雪花样浮游物,虹膜无后粘连,前房角均为宽角,房水涂片找到巨噬细胞,病理检查发现小梁网间隙、虹膜隐窝及晶体囊表面可见晶体皮质碎片和少量巨噬细胞,晶体囊膜和上皮呈退行性变,晶体皮质液化。抗青光眼药物治疗眼压均不能控制。结论:该病为一临床上独立存在的继发性、急性开角型青光眼。Abstract Object:To discuss the clinical features and therapy of phacolytic glaucoma,in order to arouse widely realization to this disorder,which do benefit to the early diagnosis and treatment to release the painfulness of the patients.Method:Clinical data of 7 aeute phaeolytic glaueoma paticnts were analyzed.Result:All of the 7 patients were suffered from senile cataract without history of glaucoma.The common clinical features were:sudden onset with blurred visual acuity,increase of intraocular pressure(IOP)with opacification and edema of cornea.The anterior chamber was deeper or normal with chamber angle open.No posterior synechia of iris.Small gray white or snowflake like float could be seen in the aqueous humor and macrophage could be found in the smear of the aqueous humor.Under pathologic exam,small fragment of lens cortex and small amount of macrophage could be found inside the space of trabecular network,the crypt of iris and on the surfacc of lcns capsule.The cortex of lens liquefied with degeneration of the lens epithelial and capsule.Poor effect to the IOP control with conditional anti glaucoma agents.Conclusion:Phacolytic glaucoma is one type of acute angle open glaucoma with characteristic clinical appearance.Removal of the opacified lens as early as quickly as possible is the only effective management to therapy and prevention of this disorder.
