摘  要:目的探讨钝挫伤性眼血液动力学变化并评价药物对改善脉络膜血供的疗效。方法以自由落体击伤,损伤能量为3.3J,造成灰兔单眼轻度钝挫伤模型。伤后分别用生理盐水(NS)、复方樟柳碱(CA)、妥拉苏林(TZL)治疗,1次/日连续1周。用眼血流图(ROG)检测双眼伤前、伤后即刻,1,3,7天ROG波幅值。结果伤后即刻各组伤眼ROG波幅值均降低,与伤前比较P<0.01。伤后对侧眼ROG波幅值均迅速降低,提示轻度挫伤致脉络膜视网膜循环障碍,可能是挫伤后继发性视网膜损害的重要原因之一。结论早期应用CA能有效地改善脉络膜供血。而TZL无此治疗作用。对侧眼ROG波幅降低是伤后对侧健眼视力下降的客观证据。Objective To study hemodynamic changes of ocular contusion and to evaluate the therapeutic effects of drugs on the improvement of choroidal blood supply in rabbits. Methods Mild ocular contusion was induced by freely falling body of 3.3 joules.54 rabbits (108 eyes) were randomly divided into 3 groups.The 54 traumatic eyes were treated respectively by compound anisodine(CA)tolazoline(TZL)and normal saline(NS)as control once daily for 7 d.Both traumatic eyes and fellow eyes were measured by rheoophthalmography(ROG) before injury,immediately after injury and 1,3,7 d after injury. Results The wave amplitudes of ROG were markedly reduced both in contusional and contralateral eyes after injury ( P <0.01).They recovered on 3 d in group CA and on d 7 in control group.However,the group TZL did not recover in this period ( P <0.05).All fellow eyes recovered within 3 d. Conclusions The changes of ROG suggested disturbance in the choroidal circulation caused by mild ocular contusion in both injured and fellow eyes.The choroidal blood supply could effectively be improved by using CA in the early stage after trauma,but not with TZL.The changes of ROG in fellow eyes caused by the consensual reaction that is an objective evidence due to vision disorder of the fellow eyes.
