青光眼小梁切除术对角膜屈光力影响的研究

摘　　要：目的探讨青光眼小梁切除术对角膜屈光力的影响。方法使用角膜地形图仪对青光眼小梁切除手术前后的角膜形态改变进行比较分析。共检查５０例病人５２只眼，其中三角形巩膜瓣组２６只眼，长方形巩膜瓣组２６只眼。结果分别对两组手术前的角膜表面非对称指数（ＳＡＩ），角膜表面规则指数（ＳＲＩ），模拟角膜计读数（ＳｉｍＫ），角膜最小曲率值（ＭｉｎＫ），角膜表面柱镜值（ＣＹＬ）与手术后１周及１个月时的角膜ＳＡＩ、ＳＲＩ、ＳｉｍＫ、ＭｉｎＫ、ＣＹＬ等数值比较，均无显著性差异（Ｐ＞０．０５）。结论无论采用三角形巩膜瓣或长方形巩膜瓣的青光眼小梁切除术对角膜屈光力改变不大，不会造成术后散光。Objective To evaluate the influence of trabeculectomy on corneal refractive power. Methods Using topographic modeling system(TMS 2),corneal refractive changes in 50 patients (52 eyes) with acute angle closure glaucoma were analyzed before and after trabeculectomy.The patients were divided into two groups:26 eyes with triangular scleral flap,26 eyes with rectangular scleral flap. Results Surface asymmetry index(SAI),surface regular index(SRI),simulated keratecmetry(SimK),minimum keratometry value(MinK) and cylinder(CYL) had no significant difference in two groups preoperation and one week,one month postoperation. Conclusion Trabeculectomy has no effect on corneal refractive power and causes no astigmatism.
