摘 要:目的 总结颅内后循环动脉瘤的CT血管成像特征,探讨CTA诊断后循环动脉瘤的价值.方法 回顾性分析26例颅内后循环动脉瘤的CTA资料,观察后循环动脉瘤的分布、形态、大小及伴随征象.结果 本组后循环动脉瘤中基底动脉瘤15个(含基底动脉分叉部动脉瘤10个),大脑后动脉瘤8个,椎动脉瘤4个,小脑上动脉瘤和小脑后下动脉瘤各1个.其中,囊状动脉瘤26个,梭形动脉瘤3个,巨型动脉瘤2个,大型动脉瘤1个,中型动脉瘤8个,小型动脉瘤18个.破裂动脉瘤12个,2例动脉瘤血栓形成,4例伴随烟雾病,6例伴随单侧或双侧胚胎型大脑后动脉.结论 颅内后循环动脉瘤的发生部位以基底动脉分叉部最常见,常伴有Willis环变异.CTA是无创性检出后循环动脉瘤的有效方法.Objective To summarize CT angiography (CTA) appearances of intracranial posterior circulation aneurysms, and to explore the value of CTA in the detection of posterior circulation aneurysms. Methods CTA materials of intracranial posterior circulation aneurysms in 26 cases were retrospectively analyzed. The distribution, morphology, size and associated appearances of poste- rior circulation aneurysms were observed. Results There were 15 basilar aneurysms (including 10 basilar bifurcation aneurysms), 8 posterior cerebral artery (PCA) aneurysms, 4 vertebral artery aneurysms, 1 superior cerebellar artery aneurysm and 1 posterior in- ferior cerebellar artery aneurysm. The aneurysms were classified into 26 saccular aneurysms, 3 fusiform aneurysms, 2 giant aneu- rysms, 1 large aneurysm, 8 middle aneurysms and 18 small aneurysms, according to their morphology and size. In this group cases, 12 ruptured aneurysms, 2 aneurysm with intraluminal thrombus, 4 cases moyamoya disease and 6 cases unilateral or bilateral fetal-- type PCA were discovered. Conclusion The most common site of intracranial posterior circulation aneurysms is at the basilar bifur- cation and often associated with variations in the circle of Willis. CTA is a noninvasive and efficient modality for posterior circulation aneurysms detection.
