摘 要:目的观察5.0%盐酸、50%乙酸及无水酒精肝内注射所产生的作用范围以及对肝、肾功能的影响,评价其应用前景。方法随机选择30只日本大白兔分为5.0%盐酸注射组5、0%乙酸和无水酒精注射组,每组10只;直视下肝内各注射1mL,1、3和7d后检测血清ALT、AST、BUN、Cr变化,术后7d处死动物,观察肝组织坏死情况及范围。结果盐酸、乙酸及无水酒精注射后均导致肝组织凝固性坏死,局部坏死区平均直径分别为(2.8±1.3)cm(、1.6±0.5)cm和(0.9±0.3)cm,肝组织坏死范围差异均有统计学意义(P<0.05)。注射后1周内三组动物ALT、AST均有升高;乙酸注射组BUN和Cr也有升高,盐酸和酒精注射组术后BUN和Cr变化不明显。结论盐酸较乙酸及无水酒精具有更强的致组织凝固力,肝组织坏死的形态较规则,边界更清晰,且无明显毒副作用,可能成为理想的瘤内注射制剂。Objective To evaluate the therapeutic efficacy and safety of hydrochloric acid,local acetic acid local injection on hepatic tissue compared to absolute ethanol injection.Methods 30 Japanese rabbits were randomly divided into 3 groups :each group had 10 rabbits and was treated with local injection of 5.0% hydrochloric acid,50% acetic acid and absolute ethanol respectively.The treatment was performed under direct visualization with 1ml liquid.Serum aspartate aminotransferase(AST),alanine aminotransferase(ALT) and blood urea nitrogen(BUN),creatinine(Cr) were detected at the 1st,3rd and 7th day after treatment.All rabbits were sacrificed at the 7th day after treatment,and mean diameter of necrotic area were measured and compared among the groups.Results The mean diameter of necrotic area of 3 groups were(2.8±1.3)cm,(1.6±0.5)cm and(0.9±0.3)cm respectively.Significant differences were found in the mean diameter of necrotic area(P0.05) among 3 groups.Necrotic area of hydrochloric acid injection was significantly larger than those of acetic acid and ethanol injection.Compared to pretreatment,serum ALT and AST elevation developed in both groups and returned to normal level at the 7th day after treatment.In 3 groups,serum BUN and Cr level also elevated at the 1st~3rd day after treatment and returned to normal level in one week.No such change was observed in hydrochloric acid and ethanol injection group.Conclusion 5.0% hydrochloric acid is more effective in the destruction of hepatic tissue than acetic acid and absolute ethano1.No main side effects in liver and kidney were found,The 5.0% hydrochloric acid may be an ideal effective endogenetic tumor coagulator.
