摘　　要：目的:探讨SmartClip自锁矫治器对牙列拥挤拔牙矫治病例在排齐整平阶段牙弓宽度及长度的影响。方法:选择使用SmartClip自锁矫治器和传统结扎式矫治器的中重度牙列拥挤患者各24例,分别作为实验组和对照组,两组均采取拔除上下颌4颗第一前磨牙的矫治方法。对两组样本治疗前及上颌牙列排齐整平后取记存模型进行测量,测量结果使用SPSS15.0统计软件进行分析。结果:排齐整平前后自锁组和传统结扎组上颌尖牙间宽度、上颌第二前磨牙间宽度、磨牙间宽度及全段牙弓长度均无显著变化;前段牙弓长度明显增加(P<0.05),但两组间变化值比较无统计学差异。结论:对于中重度拥挤拔牙病例,SmartClip自锁矫治器与传统结扎式矫治器相比,无明显扩大牙弓宽度和长度的效果,牙列排齐的机制是尖牙的远中移动提供排齐间隙。Objective：To clarify the effects on dental arch width and length by SmartClip self-ligating appliance in correction of medium or severe dental crowding with extraction approach during the initial aligning and leveling period. Methods：The sample consisted 24 patients treated by SmartClip self-ligating appliance and 24 patients by Gemini conventionally ligating appliance. All of them were treated by extraction of four first premolars. The dental casts were analyzed before and after the upper arch crowding correction. All data were analyzed by SPSS15.0 software. Results：In the two groups,the upper inter-canine width,the upper inter-second-premolar width,the upper inter-molar width and the upper whole arch length had no significantly changes. The upper anterior arch length was obviously increased（P0.05） ,but the changes from T1 to T2 were not statistically significant. Conclusion：For the extraction patients with medium or severe dental crowding,SmartClip self-ligating appliance did not have more effected changes in dental arch width and length than the conventional-ligating appliance.The mechanisms of alignment was the distal movement of canines to provide spacing for the alignment of incisors.
