摘  要:目的探讨睾酮对男性抑郁症患者临床症状及认知功能的影响。方法将42例未接受药物治疗的男性抑郁症患者按血清睾酮浓度分组,睾酮浓度〈3.0ng/mL者为异常组,3.0~106ng/mL者为正常组。两组均于治疗前评定HAMD24的总分及各项因子分,并进行WCST测验,记录各项成绩,对结果进行统计学分析。结果血清睾酮水平正常组的HAMD24焦虑因子分明显高于异常组,而迟缓因子分明显低于异常组,其他因子分两组间结果的差异没有统计学意义;两组患者血清睾酮浓度与HAMD24迟缓因子分均呈负相关关系,与HAMD24焦虑躯体化因子分均呈负相关关系。血清睾酮浓度正常组WCST测验分类数显著高于异常组,而错误应答数及持续错误数则显著低于异常组。异常组血清睾酮浓度与HAMD24认知障碍因子分呈负相关。正常组与异常组男性患者血清睾酮浓度与WCST测试的总应答数均呈正相关关系,异常组睾酮浓度与WCST的错误应答数及持续错误数均呈负相关关系。结论男性抑郁症患者血清睾酮浓度对疾病的部分临床症状及认知功能有明显影响。Objective Discuss the influence of testosterone on male depression patients in regard to their clinical symptoms and cognitive function.Methods 42 male depression patients who didn't receive drug treatment were grouped according to their serum testosterone level.Those with testosterone levels 3.0ng/mL were abnormal group,and 3.0~106ng/mL were normal group.Both groups' total score and factor scores of HAMD24 were assessed before treatment,and WSCT test was conducted and results recorded.The results were statistically analyzed.Results Serum testosterone level normal group had significantly higher HAMD24 anxiety factor score than the abnormal group;the normal group had significantly lower retardation factor score than the abnormal group.Other factors were not significantly different.Both groups' serum testosterone level was negatively correlated with HAMD24 retardation factor score;serum testosterone concentration level was negatively correlated with HAMD24 anxiety factor score.WCST test classification score in the normal group is significantly higher than the abnormal group,and their response error and the continuing response errors were significantly lower than the abnormal group.In the abnormal group,serum testosterone level was negatively correlated with HAMD24 cognitive impairment factor.In both groups,serum testosterone was positively correlated with WCST test overall response.In the abnormal group,testosterone level was negatively correlated with both WCST response error and continuing response errors.Conclusion Male depression patients' serum testosterone level has significant influence on some of their clinical symptoms and cognitive function.
