血管源性脑水肿大鼠血管周围间隙扩张的病理学观察

摘　　要：目的观察血管源性脑水肿大鼠脑组织中血管周围间隙(VRS)扩张情况,并探讨其机制。方法选择造模成功的20只SD雄性大鼠,分为模型组和对照组,每组各10只。模型组大鼠腹腔注射盐酸去氧肾上腺素6.0mg/kg,8 h注射1次,共注射3次。对照组大鼠腹腔注射等量生理盐水。所有大鼠在48 h后,取脑行冷冻切片,HE染色。在光学显微镜下观察VRS扩张情况,并行VRS计数。结果模型组大鼠注射药物后出现小便失禁,呼吸急促,竖毛肌收缩,眼球突出,肢体抽搐等。光学显微镜下显示,对照组大鼠未出现明显扩张的VRS;模型组大鼠出现明显扩张的VRS。与对照组比较,模型组大鼠扩张的VRS数目明显增多,差异有统计学意义(P<0.01)。结论血管源性脑水肿可能是VRS扩张的机制之一。Objective To observe dilation of Vichow-Robin Spaces（VRS） in the rat model of vasogenic brain edema,and investigate the mechanisms of the dilated VRS. Methods Twenty SD male rats were divided into two groups： model group and control group （n = 10 each）. In model group, all rats were injected intraperitoneally with phenylephrine hydroehloride （6. 0 mg/kg, q8 h, for three times）, while control group rats were injected with saline. After 48 h,all rats were executed under anesthesia with chloraldurat. The brain tissues were sliced into sections with a freezing microtome after perfusion fixation and observed under light microscope after HE staining. The features of VRS were observed and quantified by Mias 2000 image analysis system. Results There were many significant symptoms in model group after injection, such as urinary incontinence, shortness of breath,contraction of erector muscle, eyeball proptosis, limb twitch. Under the optical microscope, model group showed apparently dilated VRS, tissues around VRS were pale, crumbly and lightly stained, some even had sponge-like edema,while the control group was not the case. Compared with the control group,the numbers of dilated VRS in modol group were notably increased （P 〈0.01）. Conclusion As blood brain barrier is damaged by sharply increased blood pressure,tissue fluid is collected in the VRS,therefore,the VRS expands and the brain swells.
