CD83、CD1a标记肿瘤浸润性树突状细胞在大肠癌中的表达及其临床意义

摘　　要：目的探讨肿瘤浸润性树突状细胞(TIDC)在大肠癌组织中的表达及其对预后的意义。方法收集≥5年生存组和<3年死亡组大肠癌病例各30例,免疫组化和流式细胞术分别检测大肠癌组织内TIDC的CD83、CD1a表达状况,分析其与大肠癌临床病理特点和预后的关系。结果 CD83、CD1a的表达与大肠癌的分化程度、淋巴结转移无关,与Dukes分期和预后相关。免疫组化检测显示,60例大肠癌组织中19例检测到CD83阳性表达,24例检测到CD1a阳性表达。CD83、CD1a的表达随着Dukes分期的升高而降低,≥5年生存组表达高于<3年死亡组,差异有统计学意义(P<0.05)。流式细胞术检测到低水平CD83(60/60)、CD1a(60/60)和Ki-67(60/60)表达。在大肠癌组织的不同Dukes分期间,CD83、CD1a在A期+B期大肠癌组织中的表达高于C期+D期大肠癌组织,差异有统计学意义(P<0.05);CD1a在≥5年生存组表达高于<3年死亡组,差异有统计学意义(P<0.05)。结论大肠癌组织中TIDC的特征性标记物CD83、CD1a的表达与预后相关,可以作为检测大肠癌预后的参考指标。Objective To explore the expression of tumor-infiltrating dendritic cells（TIDC） in colorectal carcinoma and its prognostic significance.Methods Thirty cases of colorectal cancer patients were selected from ≥5-year survival group and 3-year death group,respectively.Immunohistochemical staining and flow cytometry were used to detect the expression of CD83 and CD1a of TIDC in colorectal cancer tissues;the relationship of the expression with clinico-pathological features and prognosis of colorectal cancer was analyzed.Results The expression of CD83 and CD1a had no correlation with differentiation degree of colorectal cancer,lymph node metastasis,Dukes staging or prognosis.Immunohistochemistry showed that 19 patients with CD83 positive expression and 24 with CD1a positive expression were detected in 60 cases of colorectal cancer.The expression of CD83 and CD1a decreased with Dukes staging increase;the expression was significantly higher in ≥5-year survival group than in 3-year death group（P0.05）.Flow cytometry detected low level of CD83（60/60）,CD1a（60/60）,and Ki-67（60/60） expressions.During different Dukes staging of colorectal carcinoma tissues,CD83 and CD1a had a significantly higher expression in A＋B phase of colorectal cancer tissues than in C＋D phase（P0.05）.CD1a had a significantly higher expression in ≥5-year survival group than in 3-year death group（P0.05）.Conclusion The expression of CD83 and CD1a,TIDC characteristic markers in colorectal carcinoma,is related to the prognosis;therefore,it can be used as a reference indicator to detect the prognosis of colorectal carcinoma.
