泛影葡胺在粘连性肠梗阻中的诊治作用

摘　　要：目的总结并评价泛影葡胺在粘连性肠梗阻中的诊治作用。方法回顾性分析386例粘连性肠梗阻患者,经胃管注入76%泛影葡胺后,分别于2h、4h、6h、12h及24h行腹部站立X线摄片,观察造影剂在肠道的运行情况及是否到达盲肠,同时密切观察患者腹部情况变化。结果泛影葡胺到达结肠时间:2h8例,4h96例,6h84例,12h115例,24h63例,24h后泛影葡胺仍未到达结肠者20例;行非手术治疗282例,有279例临床治愈,3例病情恶化,自动出院;行手术治疗104例,有102例临床治愈,2例病情加重,自动出院。结论口服泛影葡胺对肠梗阻部位、性质的诊断以及手术时机的选择具有重要的意义,同时对梗阻也有较好的治疗作用。Objective To summarize and estimate the effect of urografin in diagnose and treatment of adhesive intestinal obstruction.Methods The total of 386 patients of adhesive intestinal obstruction were analyzed retrospectively.Urografin was infused by gastric tubes,then adopt abdomen X-ray photograph to observe the running of contrast agent in intestinal tract after 2,4,6,12 and 24h,meanwhile closely observe the change of abdomen.Results According the reaching time to colon,there were 6 groups,8 cases in 2h,96 cases in 4h,84 cases in 6h,115 cases in 12h,63 cases in 24h,20 cases after 24h.There are 282 cases adopting non-operation,including 279 cases clinic curing,3 cases getting worse to leave hospital.While there are 104 cases adopting operation,including 102 cases clinic curing,2 cases getting worse to leave hospital.Conclusion Taking urografin orally or gastric tubes is very effective in diagnosing intestinal obstruction and selecting the best time for operation,meanwhile it has positive therapeutic action in intestinal obstruction.
