介入治疗肾血管平滑肌脂肪瘤破裂出血的疗效

摘　　要：目的探讨肾血管平滑肌脂肪瘤破裂出血的介入治疗疗效。方法对5例超声和CT诊断为肾血管平滑肌脂肪瘤破裂出血的患者,将导管超选择至肿瘤供血动脉,以弹簧圈栓塞肿瘤的破裂部分。对于肿瘤的未破裂部分,首先注入适量碘化油,再以数枚弹簧圈栓塞其主干。结果 5例患者均栓塞成功,出血停止。随访3个月～7年,均未再次发生出血,复查肾功能正常;肿瘤平均直径从(8.7±2.6)cm缩小至(5.3±1.6)cm。结论介入治疗肾血管平滑肌脂肪瘤破裂出血是一种安全、有效、微创的治疗方法,应作为首选治疗方法。Objective To observe the curative effect of interventional therapy for ruptured renal angiomyolipoma.Methods Five patients with ruptured renal angiomyolipoma diagnosed with US and CT underwent transcatheter renal artery embolization.The catheter was inserted into the feeding arteries of the tumors,and steel coils were used for the ruptured part of the tumors.Lipiodol was injected into the unruptured part of the tumors first,and then steel coils were used to embolize the tumors thoroughly.Results Five patients all were embolized successfully,then hemorrhage disappeared.Renal function was normal and there were on hemorrhage in the followed-up 3 months to 7 years.The mean tumor size was from（8.7±2.6）cm to（5.3±1.6）cm.Conclusion Interventional therapy for ruptured renal angiomyolipoma is safe,effective and minimally invasive,should be taken as the first choice for treatment of ruptured renal angiomyolipoma.
