复方黄柏液保留灌肠治疗溃疡性结肠炎疗效观察

摘　　要：目的:评价复方黄柏液保留灌肠治疗溃疡性结肠炎的疗效。方法:采用随机、平行阳性对照的临床试验方法,将符合入选标准的60例病人进行配对后随机分为两组,治疗组30例,对照组30例。两组病人均口服柳氮磺吡啶片1.0g,4次/d。在此基础上,治疗组加用复方黄柏液100mL保留灌肠,每日1次,治疗15天后改为隔日1次,30天后改为每周2次,总疗程3个月;对照组加用生理盐水100mL保留灌肠,灌肠用法同治疗组,总疗程也是3个月;按治疗方案治疗后,通过结肠镜检进行结肠炎症评分,共分为四个等级。并判断结肠炎症疗效,对两组出现不良反应的情况进行描述。再利用T检验及χ2检验对两种治疗方法的疗效进行比较。结果:总有效率试验组83.3%(25对/30对),对照组46.6%(14对/30对),经采用精确概率法检验得P=0.013<0.05,两组总有效率差异有统计学意义。不良反应情况:两组病人均仅出现恶心等胃肠道反应,其中试验组出现3例,对照组4例,无其它不良反应。结论:复方黄柏液保留灌肠治疗溃疡性结肠炎有良好的疗效,使用方便,无明显不良反应,值得进一步研究与推广。Objective：To evaluate the therapeutic effects of retention enema with Compound Phellodendron liquid in the treatment of ulcerative colitis （UC） . Methods：A randomized,parallel-positive-controlled clinical trials was used to deal with these treatments,60 patients were randomly divided into two groups：30 in treatment group and 30 in the control group. All patients in the two groups both orally took Sulfasalazine tablets （SASP） 1. 0g four times every day for 3 months. In the treatment group, 100mL Compound Phellodendron liquid were given via retention enema,one time every day for15 days,then one time two days for 30 days ,later the treatment was changed to two times a week for 45 days; While in control group the patients were given via retention enema with normal saline instead of Compound Phellodendron liquid,the method and the total course of treatment were as the same as those in the treatment group. Mark the colitis disease with colonoscope and pathological section,the scores were divided into four grades. And determine the efficacy of the treatment to the UC,describe the adverse reactions in two groups. T test and X2tests were used to compare the efficacy of two methods on UC. Results：The total effective rate of the treatment group was 83. 3% （25 pairs / 30 pairs） ,the control group was 46. 6% （14 pairs / 30 pairs） ,by Fisher＇s exact test P = 0. 013 0. 05,the total effective rate between the two groups was statistically significant. Adverse reactions：There were no other adverse reactions but nausea and other gastrointestinal reactions occur both in treatment group and control group,of which 3 cases in the treatment group while 4 cases in the control group. Conclusion：Retention enema with Compound Phellodendron liquid for patients with ulcerative colitis （UC） has good effect. It is an effective and convenient with slight side-effects for controlling ulcerative colitis （UC） . So it is worthy of further study and clinical application extensively.
