经直肠彩色多普勒超声鉴别卵巢肿瘤良恶性的诊断价值

摘　　要：目的　探讨经直肠彩色多普勒超声（ＴＲＳＣＤ） 诊断卵巢肿瘤的临床意义，评价ＴＲＳＣＤ 鉴别良恶性卵巢肿瘤的诊断价值。方法　采用经直肠多普勒超声（ＴＲＳＣＤ） 对７６ 例卵巢肿瘤行彩色多普勒血流显像研究，并与腹部彩色多普勒超声（ＴＡＳＣＤ） 检查结果进行比较。结果　卵巢良恶性肿瘤的血流信号显示率在ＴＲＳＣＤ 中比ＴＡＳＣＤ提高３０２ ％ ，具有极显著差异（ Ｐ＜ ０ ．００１）；ＴＲＳＣＤ 对肿瘤血流信号显示的准确性较ＴＡＳＣＤ 明显提高。ＴＡＳＣＤ与ＴＲＳＣＤ对卵巢良性肿瘤的诊断符合率分别为８７１ ％ 和８３９％ ，二者无显著差异；对恶性肿瘤的诊断符合率分别为８２２ ％ 和９１１ ％ ，后者提高８９ ％ 。结论　ＴＲＳＣＤ能更准确地显示卵巢良恶性肿瘤在血流形态学上的差异，使卵巢肿瘤良恶性的鉴别诊断进一步提高，弥补了经腹超声的不足。Objective To study clinical importance of trans rectal sonograph color Doppler (TRS CD) in diagnosing ovarian tumors.Methods 76 patients were examined by TRS CD and compared with TAS CD.Results The display rates of blood current signals in TRS CD is 30.2% higher than in TAS CD,the differences is very significant ( P <0.001);It is obvious that the display of blood current signals of tumors in TRS CD is more accurate than in TAS CD.The diagnosis correspondence of TAS CD and TRS CD in benign ovarian tumors are 87.1% and 83.9%,which the difference is not significant,but in malignant tumors,are 82.2% and 91.1%,which the later has increased 8.9%.Conclusion TRS CD can get more blood information and display the differences of benign and malignant ovarian tumors in the blood current morphology more precisely,also will increase the correctness of diagnosis to ovarian tumors.TRS CD has compensated for the limitation of TAS CD. [
