伽玛刀治疗脑膜瘤疗效及其影响因素分析

摘　　要：[目的]探讨伽玛刀放射外科治疗脑膜瘤的效果及其影响因素。[方法]选取我院伽玛刀中心2007年3月～2009年3月期间85例接受伽玛刀治疗的脑膜瘤患者。根据治疗前后的临床表现、影像学资料进行评估。[结果]随访时间3～24个月,肿瘤消失14例(16.5%),缩小53例(62.3%),无变化10例(11.8%),增大8例(9.4%),肿瘤控制率为90.6%。肿瘤的病理类型、部位与体积对肿瘤控制率的高低有显著影响,而年龄、性别、手术史和边缘剂量等对肿瘤控制率影响不大。[结论]伽玛刀是一种治疗脑膜瘤安全有效的方法,是外科手术的有益补充。尤其是颅底深部、难以手术全切除,或术后残留、复发,或年老体弱不宜手术者,首选伽玛刀可以获得理想的疗效。[Objective]To explore the effect of gamma knife radiosurgery treatment in meningiomas and its influencing factors.[Methods]From March 2005 to March 2005,our Gamma Knife Center was performed in 85 patients with meningiomas.Follow-up imaging studies and clinical manifestation were reviewed for evaluation.[Results]3-24 months after the gamma knife radiosurgery,tumor clearance was noted in 14 patients（16.5%）,tumor shrinkage in 53 patients（62.3%）,tumor stabilization in 10 patients（11.8%）,and enlargement in 8 patient（9.4%）.The tumor growth control rate was 90.6%.Statistical analysis indicated that the tumor growth control rate was significantly correlated with tumor type,location and volume（P﹤0.05）.But it did not correlate with age,gender,marginal dose,and surgical history.[Conclusion]Gamma knife radiosurgery was a safe and effective treatment for intracranial meningiomas.It was an important adjuvant therapy for surgical removal meningiomas.This treatment was especially fitted for skull base meningiomas,unresectable meningiomas,postoperative tumor residual and recurrent tumor.
