HRCT双期扫描法诊断急性胰腺炎的实验研究

摘　　要：目的：探讨ＨＲＣＴ 双期扫描对急性胰腺炎不同病理变化区的诊断价值。材料和方法：对４ 条实验犬，于注射造影剂后２３ 秒～３５ 秒（ 动脉期） 和４５ 秒～８５ 秒（ 静脉期） 在ＨＲＣＴ 上行双期扫描。比较动脉期、静脉期和双期扫描对各病变区增强特征反映的准确度。结果：双期扫描法对正常区、水肿区和坏死区判断正确率均达１００ ％ 。单期动脉期对正常区、水肿区和坏死区判断正确率分别为８３ ．３３ ％ 、５８ ．３３ ％ 和６６ ．６７ ％ 。单期静脉期对正常区、水肿区和坏死区判断正确率分别为７５ ％ 、５０ ％ 和１００ ％ 。其中动脉期将水肿区误判为坏死区以及静脉期将水肿区误判为正常区具有统计学意义（Ｐ＜ ０ ．０５） 。结论：双期ＣＴ 扫描方法可提高急性胰腺炎的诊断准确度。Purpose:To investigate the diagnostic value of double phase contrast enhanced scan protocol on high resolution CT in acute pancreatitis.Materials and Methods:23s～35s(arterial phase)and 45s～85s(venous phase)after contrast injection,4 experimental dogs were scanned by HRCT,to compare the accuracy between arterial-phase,venous-phase and double-phase scan protocol.Results:The accuracy of double phase scan protocol in judging normal,edematous and necrotic areas is 100% respectively,83.33%,58.33% and 66.67% respectively for arterial-phase scan protocol;75%,50% and 100%respectively for venous-phase scan protocol.Misjudging edematous for necrotic areas in arterial-phase or misjudging edematous for normal areas in venous-phase scan protocol has statistically significance(p<0.05).Conclusion:The accuracy of diagnosing acute pancreatitis could be promoted by using double-phase CT scan protocols.
