肝动脉化疗栓塞联合^(32)磷内照射治疗原发性肝癌

摘　　要：目的:探讨肝动脉化疗栓塞(THACE)联合^(32)磷内照射治疗原发性肝癌(PHC)的疗效及副作用。材料与方法:48例中晚期肝癌患者随机分为THACE+^(32)磷内照射治疗组(联合组,18例)和THACE组(栓塞组,30例),两组间年龄、性别、肿瘤直径及AFP比较无显著差异(P>0.05)。采用Seidinger插管,栓塞组注入阿霉素、丝裂霉素、5-氟尿啶啶及碘化油;联合组在此基础上再注入^(32)磷内照射。术后观察AFP、肿瘤大小、血象及肝功能等。结果:联合组完全缓解(CR)及部分缓解(PR)率达72.21％,栓塞组为46.66％,两组比较有显著差异(P<0.01):1年生存率分别为56.7％和30.6％(P<0.05)。两组治疗后血象均无明显改变,ALT升高较明显,但两组间无显著差异(P>0.05)。结论:THACE联合^(32)磷内照射治疗中晚期肝癌是一种新的有效的非手术疗法。Purpose: To investigate the effect of treatment of primary hepatic carcinoma by transcatheter hepatic-artery chemoembolization (THACE) combined with 32P isotope internal radiotherapy. Materials and Methods: Forty-eight cases of advanced hepatic carcinoma were randomizedly divided into two group: THACE+ 32P isotope (Group A, 18 cases and THAEC only (Group B, 30 cases). There is no signifcant difference in the two groups between age, gender, tumor diameter and AFP (P>0.05). Adriamycin, mitomycin, 5-flu-orouracil and lipiodol were injected into hepatic artery by THACE in the patients of Group B. While in the patients of Group A, additional 32P internal radiotherapy was treated. AFP, tunor diameter, peripheral white blood cells and hepatic function were dynamically observed after treatment. Results: Complete and partial remission (CR+ PR) was 72.21% in Group A and 46. 66% in Group B (P<0.01). One-year survival was 56.7% and 30.6% in Group A and Group B, respectively (P < 0. 05). After treatment there was no obvious change in number of white blood cells in two groups. As to hepatic functions, ALT was increased in both groups but no statistical difference was observed (P>0.05). Conclusions: It is a noval and effective method using THACE combined with 32P isotope internal radiotherapy to treat advanced hepatic carcinoma.
