胃癌切除胃肠短袢 Roux-en-Y 吻合术的进一步观察

摘　　要：目的:进一步评价短袢Roux-en-Y吻合术的抗返效果,探讨Roux淤积综合症的防治方法。方法:随访1999年7月至2009年6月完成的胃癌切除胃空肠短袢Roux-en-Y吻合术171例,通过症状调查、消化道钡餐、胃镜并活检、B超、CT检查确定短袢Roux-en-Y吻合术的抗返流效果和Roux淤积综合症的发病率。结果:161例(94%)获随访结果,未发现有明显症状的返流性胃炎、无倾倒综合症和Roux淤积综合症。结论:扩大胃的切除范围、同时缩短Roux袢的长度可防治Roux淤积综合症,对空肠吻合口进行抗返流加工,短袢Roux-en-Y吻合抗返流效果满意。Objective To further evaluate antire antireflux effect of Roux-en-Y procedure with a short Roux limb and to investigate the method of preventing and treating Roux stasis syndrome.Methods 171patients undergoing the Roux-en-Y reconstruction with a short limb ＇after gastrectomy for stomach cancer from july 1999 to june 2009 were foUwed up.All patients without recurrence of trmor,if only the fol- low-up data were obtained,wereevaluated,namely,the frequency of the reflux gasrites and the morbidity of Roux stasis syndrome were deter- mined.Results The follow-up data were obtained from 161 of 171 patients.Totally 21 paitents with recurring tumors were not evaluated,and of 140 remaining pateents,none had developed dumping syndrome,Roux stasis syndrome and evident alkaline reflux gastritis.Conclusion In- creasing excised size of the stomach and simultaneously shortening the length of Roux limb can prevent and treat the Roux stasis syndrome, by modifying the jejunojejunostomy Roux-en-Y anastomosis with a short limb also have excellent or good antireflux results.
