TNFR1，NF-κBp65在胃息肉及胃癌中的表达及临床意义

摘　　要：目的观察TNFR1和NF—κBp65在胃癌发生、发展中的作用以及与胃息肉和胃癌的相互关系。方法应用免疫组化法分别检测慢性浅表性胃炎、胃炎性息肉、胃增生性息肉、胃腺瘤性息肉和胃癌中TNFR1及NF—κBp65的表达。结果TNFR1在慢性浅表性胃炎、炎性息肉、增生性息肉、腺瘤性息肉及胃癌中的表达率分别为33％，45％，50％，80％，90％．胃癌组表达阳性率与其他4组比较，差异有显著性（P〈0．05），而胃腺瘤性息肉和胃癌之间差异无显著性（P〉0．05）；NF—κBp65在慢性浅表性胃炎、炎性息肉、增生性息肉、腺瘤性息肉及胃癌中的表达率分别为20％，35％，45％，70％，85％，胃癌组表达阳性率与其他4组比较，差异有显著性（P〈0．05），胃腺瘤性息肉和胃癌之间差异无显著性（P〉0．05）。TNFR1与NF-κBp65在胃癌的表达呈明显正相关（r=0．793，P〈0．01）。结论TNFR1和NF-κBp65在胃癌发生、发展中起一定作用，是反映胃癌生物学行为和预后的重要标记物。Objective To explore the expression of tumor necrosis factor receptor-1 （TNFRI） and NF- κBp65 in gastric polyps and gastric cancer,and to investigate the relationship between TNFRI and NF-κBp65 in the development of gastric cancer. Methods TNFR1 and NF-κBp65 were detected in chronic superficial gastritis, hyperplastic polyps, inflammatory polyps, adenomatous polyps and gastric cancer by immunohistochemical staining （ S-P methods ）. Results In the chronic superficial gastritis, hyperplastic polyps,inflammatory polyps,adenomatous polyps and gastric cancer, the expression rate of the TNFR1 was 33% ,45% ,50% ,80% ,90% respectively, and the NF-κBp65 was 20% ,35%, 45% ,70% ,and 85% respectively. The positive rates of TNFR1 and NF-κBp65 were both significantly different from gastric cancer and the other tissues （ P 〈0. 05 ） , while there was no significant difference between adenomatous polyps and gastric cancer（ P 〉0.05）. The TNFR1 expression was positively ,eorrelated to NF-κBp65 expression in gastric cancer（ r =0. 793,P 〈0.01）. Conclusion TNFR1 and NF-κBp65 play key roles in the development of gastric cancer. TNFRI and NF-κBp65 might be important markers for reflecting the biological behaviors and prognosis of gastric cancer.
