彩超在无体征乳腺癌诊疗中的应用价值

摘　　要：目的探讨高频彩超在早期无体征乳腺癌诊断和治疗中的应用价值。方法对超声发现的乳腺无体征肿块患者69例88个病灶,协助临床术前定位及术中实时引导下切除,并进行病理诊断。分析总结这些病灶的形态、边界、内部回声、钙化和后方衰减等声像图特征及彩色多普勒血流分布,血流信号表现,并与病理结果对照。结果本组88个病灶中,手术病理确诊乳腺癌4个,占总数的4.5%(4/88),不典型增生7个,约占总数的8.0%(7/88)。超声诊断无体征乳腺癌的灵敏度为75%,特异度为71%。结论高频彩超对乳腺无体征病灶的术前定位和术中实时检查方法简便、安全无创、可重复,在无体征早期乳腺癌的诊断和治疗中有其重要的实用价值。Objective To discuss the value of high frequency color Doppler ultrasonography in the diagnosis and treatment of breast carcinomas.with no physical signs.Methods Eighty-eight breast masses with no physical signs detected by ultrasound in all 69 cases were preoperatively localized and intraoperatively guided for resection and pathological diagnosis.Ultrasonograpic characteristics as shape,margin,internal echogenicity,calcification and posterior attenuation and the distribution and color Doppler flow signals were analysed and compared with pathological.Results Among 88 breast lesions,4 lesions were breast carcinomas confirmed by surgery and pathology,accounting for 4.5%（4/88） of all;And 7 leasions were atypical hyperplasias,accounting for 8.0%（7/88）.Totally,the sensitivity was 75% and the sensitivity was 71% for ultrasound in diagnosis breast carcinomas with no physical signs.Conclusion High frequency ultrasonography is simple,safe and repeatable in preoperative localizing and intraoperative real-time guiding for breast masses with no physical signs.Which is valuable in the dignosis and treatment 0f breast carcinomas with no physical signs.
