三维适形放疗同步单药化疗治疗老年局部晚期非小细胞肺癌的疗效观察

摘　　要：目的观察三维适形放疗同步单药化疗治疗老年局部晚期非小细胞肺癌(NSCLC)的临床疗效和毒副反应。方法放疗选用三维适形放疗(3D-CRT),化疗选用长春瑞滨(盖诺)单药,25mg/m2,静脉推注,d1,8,3周重复。于放疗第1天开始化疗,放疗期间共行2个周期化疗,放疗结束后再化疗2个周期。结果完全缓解(CR)21.2%,部分缓解(PR)54.5%,总有效率(RR)75.8%。Ⅲ～Ⅳ度不良反应发生率比较低。结论 3D-CRT同步单药化疗治疗老年局部晚期NSCLC疗效较好,毒副反应能耐受,安全可行。Objective To evaluate the clinical therapeutic efficacy and toxicity of three-dimensional conformal radiotherapy（ 3D-CRT） with concurrent single-drug chemotherapy（ CT） in elderly with locally advanced non-small cell lung cancer（ NSCLC） . Methods 3D-CRT was used for RT,and vinorelbine was used for CT. The patients were treated with vinorelbine 25 mg/m2 by intravenous drip on the first and eighth days, three weeks constituted a treatment course. CT was started on the first day of RT,and two CT courses was completed during the RT cycle. When RT was finished,the patients accepted two additional courses of CT. Results Complete remission（ CR） was detected in 21. 2% of the patients,partical remission（ PR） in 54. 5% ,with an overall response rate（ RR） of 75. 8% . Severe toxic reaction （ Ⅲ-Ⅳ） rate was low. Conclusions 3D-CRT with concurrent single-drug chemotherapy has good efficacy in treatment of elderly with locally advanced NSCLC,and the toxic reactions are tolerable. It is safe and feasible.
