曲安奈德注射致局部皮肤色素减退的对比研究

摘　　要：目的观察曲安奈德封闭治疗中出现局部皮肤色素减退的现象，探讨其发生机制及预防措施。方法将疼痛门诊治疗的病人根据镇痛封闭液的不同分为泼尼松龙组和曲安奈德组，比较其治疗后局部皮肤色素减退的发生率。结果泼尼松龙组未发现治疗后出现局部皮肤色素减退，曲安奈德组表浅部位注射后2～3周部分病例出现局部皮肤色素减退，深部注射未出现此现象。色素减退多见于育龄期妇女。结论曲安奈德封闭治疗可能出现局部皮肤色素减退，曲安奈德溢出引起了局部皮肤色素减退，注射后出针时局部按压可减少其发生。临床治疗时应事先告知患者并发色素减退的可能性。Objective To observe the local cutaneous depigmentation after triamcinolone acetonide injection and investigate the mechanism and precaution of depigmentation. Methods Outpatients were divided into prednisolone group and triamcinolone acetonide group according to the injection medicine. The incidence rate of local cutaneous depigmentation was compared. Results Local cutaneous depigmentation was observed among some patients in triamcinolone aeetonide group when given at a superfcial site 2 -3 weeks after injection. Local skin discoloration was not found in prednisolone group,nor was it when triamcinolone acetonide was injected deeply. Depigmentation mostly happened among women in child -bearing period. Conclusions Treatment with triamcinolone aeetonide injection may cause local cutaneous depigmentation, leakage of triamcinolone acetonide resulted in depigmentation and the adverse effects of injection may be prevented by applying pressure over the injection site as the needle is withdrawn to prevent leakage of cortieosteroid along the needle track. Patients should have been forewarned about the possibility of depigmentation.
