摘 要:目的探讨泪小管疾病在超声生物显微镜(ultrasound biomicroscope,UBM)图像中的成像特点,为泪小管疾病的诊治提供影像学依据。方法选取2008年1月至2009年3月因泪小管疾病就诊于武警总医院眼科的患者22例(25眼),其中慢性泪小管炎10例(10眼),先天性泪小点、泪小管缺如3例(6眼),陈旧性泪小管断裂8例(8眼),泪小管内肿物1例(1眼),均行UBM检查,观察各种泪小管疾病在UBM图像中的成像特点。结果慢性泪小管炎的UBM检查结果显示,病变泪小管管腔明显扩张,部分扩张的管腔内可见点状中回声。1例泪小管肿物的大小、形状在UBM检查中可清楚成像,表现为中回声、密度均匀的球状肿物。3例先天性泪小管缺如患者,1例可见下泪小管管腔样结构。8例陈旧性泪小管断裂病例,7例探查到泪小管鼻侧断端,1例未探查到断端。结论 UBM用于泪小管疾病的检查可行性好,但本研究只是UBM检查在泪小管疾病诊治中的初步应用,其准确性和灵敏性以及仪器改进等方面仍需进一步研究。Objective To study the imaging characteristics of ultrasound biomicroscope(UBM)in canalicular diseases and provide the imaging basis for their diagnosis.Methods Twenty-two patients(25 eyes)with canalicular diseases from January 2008 to March 2009 in Armed Police General Hospital were chosen,including 10 cases(10 eyes)of chronic canaliculitis,1 case(1 eye)of canalicular mass,3 cases(6 eyes)of congenital canaliculus absence,8 cases(8 eyes)of old lacerated canaliculus.All patients underwent the examination by UBM,and the imaging characteristics of various canalicular diseases were observed.Results UBM imaging of chronic canaliculitis showed ectasia of the canaliculus and sulfur grains could be seen in some ectatic canaliculus.The shape,dimension,texture of mass in the canaliculus were explicitly showed in UBM imaging,which showed the globe with middle echo and well-distributed density.The tube-shaped structure in the inferior position could be seen in 1 patient with congenital canaliculus absence.Meanwhile,8 cases with old lacerated lacrimal canaliculus,the nasal lacerated part in 7 cases were found and 1 case was failed.Conclusions It is practicable to check canalicular diseases by UBM.However,this test aims at the initial application of UBM for the diagnosis of the canalicular diseases.Its accuracy,sensitivity and improvement of instrument need further study.
