乳腺导管内乳头状瘤病的MR影像学表现

摘　　要：目的分析乳腺导管内乳头状瘤病（BIDP）的MR表现特征，探讨MR对该病的诊断价值。方法回顾性分析经手术和病理证实的18例乳腺导管内乳头状瘤病的MR影像学表现，包括病变形态学、信号强度、肿块内部强化特征及动态增强曲线类型。并将大体病理所见与MR表现相对照。结果18例BIDP中肿块性病变15例，其中13例为单发肿块，2例为多发肿块。平扫去脂T2WI呈均匀稍高信号8例，高信号2例，混杂稍高信号5例；病灶圆形边缘光滑7例，卵圆形边缘光滑1例，分叶状边缘不规则1例，分叶状边缘毛刺4例，沿导管走行方向分布多发结节及肿块2例。增强内部均匀强化5例，不均匀强化5例，边缘强化5例；强化曲线为Ⅱ型8例，Ⅲ型7例。18例BIDP中2例为段样非肿块样病灶，去脂T2WI呈稍高信号，增强内部不均匀强化，其中1例合并乳头内陷，强化曲线均为Ⅲ型。18例BIDP中1例呈全乳腺非肿块样病灶，去脂T2WI呈稍高信号内见手指样低信号延伸至乳头，增强内部特征呈网状树枝状强化，强化曲线为Ⅱ型。结论MRI对发现与定位BIDP病灶及术前评估具有重要意义。Objective To explore the MR imaging manifestations of breast intraductal papillomatosis （BIDP},so that to improve the radiologic diagnostic ability of this disease. Methods MR imaging data of 18 cases with intraductal papillomatosis of breast con- firmed by pathology were analysed retrospectively. Results Of the 18 cases,there were mass--like lesions in 15,including mono-- mass in 13 and multi--mass in 2,the lesions were uniformity slightly high signal in 8, high signal in 2 and uneven high signal in 5 on T2 WI fat saturation sequence（ T2 WI FSE）, the lesions were spherical with smooth glossy in 7, orbicular--ovate with smooth glossy in one,lobulated with irregular rim in one,irregular with barb rim in 4 and nodules and masses along duct distribution in 2, after administration of contrast agent, the lesions were uniform enhanced, uneven enhanced and borderline enhanced in 5, respectively, and the time intensity curve （TIC） appeared as type Ⅱ in 8 and type Ⅲ in 7. There were segmental--like lesions in 2 cases,which were slight hyperintense on T2 WI FSS and appeared as uneven enhanced, one case accompanied with retracted nipple and TIC was type Ⅲ. Of the 18 cases,there was non--mass--like lesions involving whole breast in one, the lesion was slight hyperintense on T2WI FSS with mesh and branch enhancement,the TIC was type Ⅱ. Conclusion The foci of BIDP can be detected and localized with MR imaging accurately, which is of important value in evaluation of the disease preoperatively.
