摘  要:目的:评估引导组织再生技术在显微根尖周手术中的效果。方法:选择患有慢性尖周炎、尖周稀疏区直径>6 mm的病人44例共71个患牙,随机分为实验组和对照组。实验组在完成显微根尖手术后于骨腔内植Bio-Oss骨替代材料,覆盖Bio-Gide生物膜;对照组只接受显微尖周手术治疗。分别于术后1、3、6、9、12个月进行临床和X线复查,对复查结果进行评估。结果:术后12个月时,实验组22例中21例(95.45%)被判定为成功,1例(4.55%)失败,对照组21例中19例(90.48%)被判定为成功,1例(4.76%)失败,两组间成功率无显著性差异(P>0.05)。实验组在6个月、9个月时X线评估被确认为完全愈合和不完全愈合的病例明显多于对照组,且均具有显著性差异(P<0.05);12个月时两组间的X线评估无显著性差异(P>0.05)。结论:在显微根尖周手术时应用引导组织再生技术没有显著提高其远期成功率,但加快了根尖周骨质破坏区的愈合速度。AIM: To evaluate the effects of guided tissue regeneration(GTR) technique in apical microsurgery.METHODS:Seventy-one teeth in 44 patients were included in this study,and were randomly divided into the experimental group and control group.Apicomarginal defects was grafted with Bio-Oss bone material and covered with Bio-Gide membrane after the apical microsurgery in the experimental group,while patients in the control group received a simple apical microsurgery only.Periapical healing was assessed clinically and radiographically at 1,3,6,9,12 months after surgery.RESULTS:At 12-month after surgery,21 cases(95.45%) of 22 patients in the experimental group were considered success and 1 case(4.55%) failed.And 19 cases(90.48%) in the control group were considered success and 1 case(4.76%) failed.There was no significant difference between the two groups(P0.05).However,at 6-month and 9-month post-surgery,cases in the experimental group classified as "complete healing" and "incomplete healing" according to a previously published classification by radiographical assessment were much more than the control group(P0.01 and P0.05).At 12-month,the radiographical assessment between cases of the two groups showed no significant difference(P0.05).CONCLUSION:Although GTR in apical microsurgery did not improve the long-term success rate significantly,it accelerated the healing rate of apicomarginal defects.
