摘 要:目的探讨p53结合蛋白1(53BP1)在前列腺腺癌及癌旁良性前列腺组织、前列腺高级别上皮内瘤变、良性前列腺增生中的表达及其与临床病理的关系和意义。方法对50例前列腺腺癌根治标本的癌及癌旁良性前列腺组织、20例前列腺高级别上皮内瘤变、20例良性前列腺增生标本应用免疫组织化学EnVision两步法检测53BP1在以上病变组织中的表达,并探讨其与前列腺腺癌发生及临床病理的相关性。结果 53BP1蛋白在良性前列腺增生上皮、高级别上皮内瘤变、癌旁良性前列腺上皮、前列腺腺癌细胞中阳性表达呈递减趋势,且在良性前列腺增生上皮、高级别上皮内瘤变、癌旁良性前列腺上皮的表达显著高于前列腺腺癌细胞(P=0,0.015,0),在癌旁良性前列腺上皮及高级别上皮内瘤变中的表达显著低于良性前列腺增生上皮(P=0,0.035),而癌旁良性前列腺上皮与高级别上皮内瘤变的差异无统计学意义(P=0.658)。53BP1在低年龄组中表达明显低于高年龄组(P=0.001),而与其他病理参数无关。结论 53BP1作为抑癌基因,其表达的缺失与前列腺腺癌发生有关,它有可能成为前列腺腺癌的早期指示因子,预示前列腺腺癌的发生可能。Purpose To study the expression of tumor protein p53 binding protein 1(53BP1) in prostate adenocarcinoma,adjacent benign prostate tissue,high grade prostatic intraepithelial neoplasia(HGPIN),benign prostatic hyperplasia(BPH) and its clinicopathologic significance.Methods Immunohistochemical study for 53BP1 was performed on paraffin sections of 50 cases of prostate adenocarcinoma and adjacent benign prostate tissue,20 cases of high grade prostatic intraepithelial neoplasia,20 cases of benign prostatic hyperplasia,and its value was discussed for tumorigenesis of prostate adenocarcinoma.Results Immounohistochemical study showed the epithelial cells of benign prostatic hyperplasia,high grade prostatic intraepithelial neoplasia,adjacent benign prostate tissue,and prostate adenocarcinoma cells were positive for 53BP1 in the nucleus,which was decreased gradually.The expression in the epithelial cells of benign prostatic hyperplasia,high grade prostatic intraepithelial neoplasia and adjacent benign prostate tissue were significantly higher than that in prostate adenocarcinoma cells(P=0.015).53BP1 expression in adjacent benign prostatic epithelial cells and high grade prostatic intraepithelial neoplasia was significantly lower than that in benign prostatic hyperplasia epithelial cells(P=0.035).However,there was no statistical differences between adjacent benign prostate epithelial cells and high grade prostatic intraepithelial neoplasia(P=0.658).53BP1 expression was much lower in ≤68-year-old group than in 68-year-old group(P=0.001).Conclusions As a tumor suppressor gene,loss of 53BP1 expression may play a role in the initiation of human prostate adenocarcinoma.It may become an indicator of early-stage prostate adenocarcinoma.
