摘 要:目的:探讨肿瘤相关巨噬细胞(tumor-associated macrophages,TAMs)在食管癌组织浸润密度、分布及对预后的影响。方法:应用免疫组化方法检测CD68阳性巨噬细胞在食管癌组织及癌旁正常组织的浸润,比较癌旁正常组织和食管癌组织中巨噬细胞浸润密度,并分析癌组织中TAMs浸润密度、分布对食管癌患者生存时间及预后的影响。结果:食管癌组织TAMs浸润密度明显高于癌旁正常组织(P<0.05)。食管癌间质和癌巢TAMs浸润高密度组5年生存率低于低密度组(P<0.05);癌间质TAMs浸润高密度组5年生存率明显低于低密度组(P<0.05);相反,癌巢TAMs浸润高密度组5年生存率高于低密度组(P<0.05)。癌间质和癌巢、癌间质TAMs浸润密度与患者生存时间均呈负相关性(P<0.05);癌巢TAMs浸润密度与患者生存时间呈正相关(P>0.05),癌间质及癌巢TAMs浸润密度是独立的预后因素(P均<0.05)。结论:TAMs在食管癌组织中具有较高的浸润密度,且不同微环境的浸润密度不同,与患者生存时间及预后具有相关性。Objective:To investigate the infiltration of tumor-associated macrophages (TAMs) in the tissue of esophageal carcinoma and its impact on patient prognosis. Methods: Immunohistochemistry was used to investigate the infiltration of TAMs expressing CD68 in esophageal carcinoma tissue and adjacent normal tissue. The density of macrophage infiltration between the esophageal carcinoma and the adjacent normal tissue was compared, and the density and distribution of macrophage infiltration in the esophageal carcinoma tissue was analyzed to find its impact on patient survival. Results: The mean density of TAMs infiltrating esophageal carcinoma tissue was significantly higher than that in normal adjacent tissue ( P 〈 0.05). Compared with patients with a low density of TAMs in stroma and cancerous tissue, the patients with a high density of TAMs had a shorter five-year survival rate ( P 〈 0.05 ). The five-year survival rate for patients with a low density of TAMs in stroma was much shorter than that for the patients who have a low density of TAMs in the same tissue ( P 〈 0.05 ). The five-year survival rate was longer for the patients with a low density of TAMs in the cancer nest than in patients with a high density of TAMs in the cancer nest ( P〈 0.05 ). The infiltration density of TAMs in the cancer stroma and nest was negatively correlated with patient survival ( P 〈 0.05 ). The infiltration of TAMs in stroma and cancer nest was independent prognostic factors affecting the patient survival ( P 〈 0.05 ). Conclusion: There is a higher density of TAMs in esophageal carcinoma than in normal esophageal tissue. The degree of TAM infiltration in different parts of esophageal carcinoma is correlated with patient survival and prognosis.
