摘 要:目的探讨缺氧诱导因子-1α(hypoxia-inducible factor-1a,HIF-1α)和血管内皮生长因子-C(VEGF-C)在贲门癌组织中的表达与意义。方法选择贲门癌组织标本60例和正常贲门组织20例,采用免疫组化SABC、PCR、荧光定量RT-PCR和基因测序等方法,定量检测HIF-1α及VEGF-C表达,并分析两者与贲门癌临床病理学特征的关系。结果 HIF-1α、VEGF-C在贲门癌组织中的表达明显高于正常组织(P<0.01)。随着癌细胞分化程度降低和TNM分期增加,HIF-1α、VEGF-C蛋白表达水平增加,且两者蛋白水平低分化与高分化、中分化组相比显著增加(P<0.01),Ⅲ~Ⅳ期较Ⅰ~Ⅱ期显著增加(P<0.05);有淋巴结转移组HIF-1α、VEGF-C表达水平比无淋巴结转移组明显增加(P<0.01),且靠近转移淋巴结的癌细胞HIF-1α、VEGF-C阳性程度高于远离转移淋巴结的癌细胞。≥1/2肌层浸润组HIF-1α、VEGF-C表达水平比<1/2组明显增加(P<0.01)。HIF-1α和VEGF-C的表达呈正相关。HIF-1α和VEGF-C表达与肿瘤大小、患者性别、年龄均无关。HIF-1α、VEGF-C mRNA表达水平在贲门癌组织中明显高于正常组织,分别增加3.76和2.14倍。基因序列分析及BLAST比对结果证实PCR产物。结论 HIF-1α及VEGF-C介导了贲门癌的发生,与贲门癌临床病理学特征关系密切,HIF-1α、VEGF-C高表达和淋巴结转移相关。Objective To investigate the expression of hypoxia inducible factor-1α(HIF-1α) and vascular endothelial growth factor-C(VEGF-C) in the cardiac carcinoma and their significance.Methods Expression and distribution of HIF-1α and VEGF-C were detected in operation tissue specimens of 60 cases of cardiac carcinoma tissues and 20 cases of normal cardiac tissue by immunohistochemistry SABC method,PCR,quantitative RT-PCR and gene sequencing methods.Relations of HIF-1α and VEGF-C with clinicalpathological characters.Results HIF-1α,VEGF-C expression in cardiac tissue was significantly higher than normal tissue(P0.01).With the reduced level of cell differentiation and TNM stage increase,HIF-1α and VEGF-C protein expression were increased.The poorly differentiated was significantly increased compared with well differentiated and moderately differentiated group(P0.01).Ⅲ~Ⅳstage expression were significant increased compared toⅠ~Ⅱ stage(P0.05).HIF-1α,VEGF-C expression with lymph node metastasis was significantly increased compared with without lymph node metastasis(P0.01).HIF-1α,VEGF-C expression in ≥1/2 myometrial invasion group than in the 1/2 was significantly increased(P0.01).HIF-1α and VEGF-C expression was positively correlated.HIF-1α and VEGF-C expression are not related with tumor size,sex and age.HIF-1α,VEGF-C mRNA expression was significantly higher than in the cardiac tissue of normal tissue,respectively 3.76 and 2.14 times.Gene sequence analysis and BLAST comparison results confirm PCR product.Conclusion HIF-1α and VEGF-C might play potentia1 roles in carcinogenesis for cardiac cancer,have affinity with clinicalpathological characters of cardiac cancer.HIF-1α and VEGF-C high expression may have mutual effect in lymph node metastasis.
