摘　　要：目的通过彩色多普勒超声(CDUS)检查复查肝细胞癌(HCC)行肝动脉化疗栓塞(TACE)后血管血流情况,探讨彩色多普勒血流技术检查技术对指导肝癌栓塞术后进行再介入治疗的价值。方法观察77例患者行143次TACE后1月左右行彩色多普勒常规二维多切面扫描和CDU血流显像(CDFI)及频谱分析肿瘤内部及周边血流情况指导再次行介入治疗。结果 77例HCC患者TACE后行CDU检查,根据其血流特点指导再介入术式为TACE、TAI、PEI和TACE联合、PEI等治疗,患者生存期明显改善。结论通过CDU技术全面了解HCC患者经TACE后肿瘤及周边的血流、血供状况,可以作为肝细胞癌患者TACE等介入治疗后的常规检查手段,对于肿瘤状况评定和再次介入治疗有着明确的指导意义。Objective To examine the blood flow after HCC transcatheter arterial chemoembolization（TACE） through CDU and to explore the guidance value of CDUS examination in the re-intervention treatment after hepatocarcinoma chemoembolization.Methods 77 patients underwent TACE 143 times.About a month afterwards,Color Doppler normal 2D multiplanar scanning and Color Doppler flow imaging（CDFI） were conducted to analyze the blood flow inside and around the carcinoma before the implementation of re-intervention treatment.Results 77 patients of HCC were examined by CDU after undergoing TACE.The features of their blood flow were taken as a guidance for the re-intervention treatment in the form of TACE,TAI,PEI combined with TACE and PEI.The survival time of the patients were significantly improved.Conclusion The technique of CDU can improve the understanding of the HCC patients＇ blood flow and supply inside and around the carcinoma after receiving TACE.It can be taken as a normal examination after HCC patients＇ TACE treatment.It has the significance of guidance for the assessment of the carcinoma state and re-intervention treatment.
