晚期肿瘤合并低钠低钾的诊疗

摘　　要：目的了解晚期肿瘤合并电解质紊乱的成因、临床特点、治疗效果及对预后的影响因素。方法通过对肿瘤内科2008年11月-2010年5月收治的57例晚期肿瘤患者的电解质情况进行检测分析,针对电解质紊乱情况进行纠正并对预后的影响因素进行分析。结果电解质紊乱患者24例(42.10%),低钾18例(75.00%),低钠15例(62.5%),其中入院时常规血清学检测诊断电解质紊乱有17例,住院过程中随病情发展,部分患者出现肢体乏力、恶液质以及进食困难等,予以复查血清电解质诊断出7例。经补钾、补钠等治疗后电解质均恢复到正常范围。但是有12例患者在肿瘤进展或化疗后再次出现电解质紊乱。结论低钠低钾在晚期肿瘤患者中存在较普遍,对肿瘤的治疗及预后有较大影响。关注晚期肿瘤患者存在的各种合并症,尽早发现并给予合适的治疗,可改善晚期肿瘤患者生活质量,延长生存时间。Objective To investigate the cause, clinical characteristics, treatment effectiveness and the influencing factor of prognostic of electrolyte imbalance in the advanced cancer. Methods From Nov. 2008 to May 2010,57 patients with the ad- vanced cancer were enrolled. The electrolyte and its effect on patient treatment and prognosis were carried out. The electrolyte imbalance was corrected and the influencing factor of prognostic was analyzed. Results There were 24 cases of disorderly electrolyte（42.10% ） , 18 low potassium（75.00% ） ,and 15 low sodium（62.5% ）. 17 patients had been to the hospital diagnosed as electrolytes turbulent, the process developed, they had difficulty in physical strength and food ; another 7 patients were diagnosed by electrolyte determination once more. The electrolyte imbalance was corrected after infusing potassium and sodium. However, 12 patients occurred electrolyte disturbance after chemotherapy or tumor progression. Conclusion Low sodium low potassium in patients with advanced cancer was very common,and which had a greater impact on treatment and prognosis of patients. Concerning the existence of the late complications, discovering as early as possible and giving rightly treating were important to improve the quality of life in patients with advanced cancer and prolong survival time.
