整合素α4和β1在胃癌中的表达及临床意义

摘　　要：目的 探讨整合素α4、β1在胃癌组织中的表达及其与胃癌临床、病理特征和预后的关系.方法 采用免疫组化Envision法检测84例胃癌组织中整合素α4、β1的表达,并对胃癌患者进行术后随访.结果 （1）本组胃癌患者α4表达率为75.0%（63/84）,β1表达率为21.4%（18/84）,正常胃黏膜均不表达.（2）整合素α4表达与胃癌是否出现远处转移和临床病理分期相关（均P＜0.01或0.05）,而与肿瘤浸润深度、Borrmann分型、组织学类型及是否伴有淋巴结转移无关（均P＞0.05）;整合素β1表达与胃癌浸润深度相关,而与其它病理学特征均无关（均P＞0.05）.（3）84例患者中共有77例获随访,随访率91.7%,随访期为3～88个月,中位生存时间16.5个月,α4表达阳性者术后生存率高于阴性者（P＜0.05）,而β1表达阳性者术后生存率低于阴性者,但差异无统计学意义（P＞0.05）.结论 整合素α4表达减少可能导致胃癌进展和远处转移;而β1表达可能与胃癌侵袭性和预后差有关,提示表达整合素α4与β1的胃癌可能具有不同的生物学行为.Objective TO investigate the expressions of intergin α4 and 13 1 in gastric carcinoma and the relation with its clinicopathologic features. Methods The expression of α 4 and 13 1 in 84 cases of gastric carcinoma was examined immunohistochemically. All samples obtained were accompanied by patient follow-up data. Results The expression rate of αβ4 was 75.0% （63/84）and β 1 was 21,4%（18/84）, while a4 and β 1 were not detected in normal gastric mucosa. Expression level of a4 was correlated with metastasis and clinical stage of gastric carcinoma （P〈0.01 or 0.05）. Expression level of β 1 was correlated with invasive ability of gastric carcinoma （P 〉0.05）. Out of 84 patients 77 were followed up （91,7%） for 3 - 88 months with a median survival time of 16.5 months, The survival rate was higher in a 4 positive group than that in negative group（P 〈0.05 ）. The survival rate was lower in 13 1 positive group than that in negative group （P 〉0.05 ）. Conclusion The results indicate that low expression level of oc 4 may lead to metastasis and progression of gastric carcinoma; while high expression level of β1 may lead to infiltrating and poor prognosis of gastric carcinoma.
