参芎葡萄糖注射液结合三维适形外照射放射治疗Ⅲ期老年非小细胞肺癌临床疗效观察

摘　　要：目的:观察参芎葡萄糖注射液结合三维适形外照射放射治疗Ⅲ期老年非小细胞肺癌临床疗效和放射治疗的毒副反应。方法:65例Ⅲ期老年非小细胞肺癌患者随机分成两组。A组:32例,应用参芎葡萄糖注射液,使用直线加速器6 MV的X射线进行纵隔区及原发病灶的三维适形外照射放疗,200 cGy.次-1,1次.d-1,5次.周-1,总剂量4 000 cGy;原发病灶三维适形放射治疗追加剂量放疗,200 cGy.次-1,1次.d-1,共10次,总剂量2 000 cGy。放疗开始当天参芎葡萄糖注射液100 ml静脉注射,1次.d-1,共6周。B组:33例,放射治疗方法同A组,放疗当天开始生理盐水250 ml加入维生素C注射液2 g、维生素B6注射液200 mg静脉滴注,1次.d-1,共6周。观察两组术后肿瘤近期疗效、生存率(术后3、6、12、18个月生存率)、免疫功能、放疗后生存质量和毒副反应。结果:A组有效(CR+PR)率为71.9%,B组为45.5%(P<0.05),两组CR率分别为46.7%、24.2%(P<0.05)。A组3、6、12、18个月生存率分别为96.88%、90.63%、71.90%、34.40%,平均生存期为(16.2±3.1)个月;B组患者术后3、6、12、18个月生存率分别为93.94%、72.73%、60.60%、27.30%,平均生存期为(13±2.3)个月;两组比较12、18个月生存率无显著差异(P>0.05)。A组近期疗效明显优于B组,免疫功能、放疗后生存质量A组明显好于B组,A组毒副反应明显较B组减轻。结论:参芎葡萄糖注射液结合三维适形外照射放射治疗Ⅲ期非小细胞肺癌,可改善老年患者的生存质量,减轻放射治疗的毒副反应。Objective： To observe the local control rate and serious toxicity on senate ligusticum glucose injection and the 3-dimensional conformal radiation therapy in elderly patients with Ⅲ non-small cell lung cancer（NSCLC）.Methods： 65 cases elderly patients with NSCLC were randomly divided into two groups： group A and group B.group A included 32cases treated with senate ligusticum glucose injection and the 3-dimensional conformal radiation therapy,group B included 33 cases with VitC,VitB6 injection and the 3-dimensional conformal radiation therapy.To observe the clinical characteristics of tumor,recent effects,toxication and side effects,survival condition（postoperative survival rate in 3,6,12 and 18 months,respectively） and so on.Results： The CR rate of group A was higher than that of group B（P0.05）.Postoperative survival rate of group A were 96.88%,90.63%,71.90% and 34.40% at 3,6,12 and 18 months;group B were 93.94 %,72.73%,60.60% and 27.30% at 3,6,12 and 18 months respectively,survival rate of 12,18 months had no significant difference in the two groups.The side effects and toxications of groups A were better than group B.Conclusion： Senate ligusticum glucose injection and the 3-dimensional conformal radiation therapy in elderly patients with Ⅲ non-small cell lung cancer can reduce the side effects and toxications,improve life quality of patients.
