上海市青浦区2002年—2008年新发胃癌患者队列生存分析

摘　　要：目的：分析上海市青浦区胃癌患者发病及生存情况,为今后的筛查和干预工作提供依据。方法：对上海市青浦区2002年—2008年新发的原发性胃癌患者随访资料进行分析,描述发病年龄、性别、部位等特点,并建立Cox比例风险模型,分析生存率的主要影响因素。结果：2002年—2008年上海市青浦区新发的原发性胃癌1 333例。发病年龄构成呈偏态分布,40岁以上发病率明显升高,发病高峰70~80岁;男性多于女性（2.14：1）;发病部位以胃体和胃窦最多见。1 333例胃癌患者中位生存时间为11.66个月,胃癌患者的1年生存率为49.75%,5年生存率为23.67%。患者的死亡危险按照部位依次（由低到高）为胃体、胃窦、胃底贲门、跨越部位、胃未特指。年龄越高,死亡危险越高,性别因素对生存率的影响无统计学意义。结论：上海市青浦区居民40岁以后胃癌的发病率明显上升,发病部位以胃体和胃窦为主,1年生存率约50%。Objective：To investigate the incidence and survival rates on follow-up data about gastric cancer patients in Qingpu district in Shanghai.Methods：The follow-up data of new gastric cancer cases between 2002-2008 in Qingpu district was investigated.Data about age at diagnosis,gender,survival time,cancer site were collected.Stata 8.0 software was used to analyze.Survival rate was analyzed with single and multiple Cox proportional hazards regression model respectively.Results： The data of 1 333 gastric cancer cases were analysed.The incidence increased since 40 years old and the peak was at 70-80 ys.The ratio of male versus female was 2.14∶1.Gastric corpus and pylori were the most common sites of gastric cancer.The median survival time was 11.66 months.One year survival rate was 49.75% and the five year survival rate was23.67%.The hazard ratio of death of patients with cancer at stomach corpus or pylori was lower than those with cancer at fundus,cardia or site unclear.Conclusions： The incidence increased after 40 years old in Qingpu district in Shanghai.Gastric corpus and pylori were the common sites for gastric cancer.The one year survival rate was about 50%.
