镍铬烤瓷牙对肾功能的影响

摘　　要：目的:探讨镍铬烤瓷牙对使用者肾功能的影响。方法:对浙江某县45例烤瓷牙安装者(接触组:镍铬烤瓷牙37例;对照组8例含钛烤瓷牙)进行问卷调查并收集尿样和血样,测定血中总蛋白(TP)、白蛋白(ALB)、球蛋白(GLB)、血肌酐(CREA)、白蛋白/球蛋白比值(A/G)、和尿中白蛋白(ALB)、尿N-乙酰-β-氨基葡萄糖苷酶(NAG)、β2-微球蛋白(β2-MG)、尿视黄醇蛋白(RBP)等肾功能指标并进行分析。结果:两样本t检验显示镍铬烤瓷牙(接触组)在血TP、ALB、GLB、白蛋白/球蛋白比值A/G、CREA和尿CREA、ALB、NAG、RBP、β2-MG等多项肾功能指标间均无明显统计学意义(P>0.1)。线性回归分析显示在控制年龄、性别、体质指数、吸烟和饮酒等因素的影响后,烤瓷牙的安装类型、时间和数量和多数血尿肾功能指标间均无明显关系(P>0.1),仅尿肌酐和安装类型显示为正相关(β=0.31,P=0.056),和安装时间显示为负相关(β=-0.37,P=0.029)。结论:本研究结果显示目前尚不能认为镍铬烤瓷牙对肾功能有损害,但结果需进一步加大样本量研究证实。Objective：To discover the impact of Ni-Cr metal-ceramic crown on renal function.Methods： 45 subjects with metal-ceramic crown（37 subjects having installed Ni-Cr metal-ceramic crown were classified exposed group,and 8 subjects having installed Ti metal-ceramic crown were classified control group） had been investigated by questionnaire,and their urine sample and blood sample had been collected.The TP,ALB,GLB,CREA in blood sample and the ALB,NAG,β2-MG,RBP in urine sample had been analyzed.Results： The t-test showed that no statistical difference existed between the TP,ALB,GLB,CREA in blood sample or the ALB,NAG,β2-MG,RBP in urine sample of exposed group and those of control group.Linear Regression showed that after controlled age,sex,body mass index,smoke and drink,no relationship was found between the metal-ceramic crown type,installed period,installed crown number and most indicators in blood samples or urine samples.The CREA in urine sample positively correlated with the metal-ceramic crown type（β = 0.31,P=0.056）,and negatively correlated with the installing period.Conclusion： This study showed that the Ni-Cr metal-ceramic crown may has no negative impact on renal function,but the conclusion should be further confirmed by the research including larger sample size.
