摘 要:目的探讨p53与p21蛋白在乙肝病毒/黄曲霉毒素双暴露原发性肝细胞癌中的表达及意义。方法运用免疫组化S-P法检测67例手术切除经病理证实为原发性肝细胞癌的肝癌组织及11例正常肝组织p53蛋白与p21蛋白的表达情况。结果 67例肝癌组织中p53和p21蛋白表达的阳性率分别为79.1%(53/67)和44.8%(30/67)。p53蛋白表达在实验组和对照组A组与B组的阳性率分别为95.2%(40/42)、20.0%(2/10)、73.3%(11/15);p21蛋白表达在实验组和对照组A组与B组的阳性率分别为45.2%(19/42)、60.0%(6/10)、33.3%(5/15)。p53蛋白表达在实验组与对照组A组、B组比较,差异均有统计学意义(P<0.05);p21蛋白表达在实验组与对照组A组和B组比较,差异均无统计学意义(P>0.05)。p53和p21蛋白在肝癌组织中的表达呈低度负相关,其相关性无统计学意义。p53与p21蛋白表达率在实验组及对照组A组和B组中,其相关性均无统计学意义。11例正常肝组织p53及p21蛋白表达均为阴性。结论在双暴露因素下的肝癌发生过程中,p53有更高的阳性表达率,且p53的表达对p21具有着促进或抑制的多重作用,提示可能为乙肝病毒和黄曲霉毒素双因素协同作用的结果。Objective To explore the expression and significance of p53 and p21 protein in hepatocellular carcinomas(HCC)that were with both hepatitis B virus(HBV)infection and aflatoxin exposion.Methods The expression levels of p53 and p21 protein were examined by immunohistochemical(IHC)S-P method in 67 cases of pathologically proved HCC,and in 11 cases of normal liver tissue which were used as comparison.Results The positive expression rates of p53 and p21 in HCC tissues were 79.1%(53/67)and 44.8%(30/67)respectively.The positive rates of p53 in experimental group,control group A and B were 95.2%(40/42),20.0%(2/10)and 73.3%(11/15)respectively.The positive rates of p21 in experimental group,control group A and B were 45.2%(19/42),60.0%(6/10)and 33.3%(5/15)respectively.The differences of p53 expression between the experimental group and the two control groups were statistical significant(P0.05).The differences of p21 expression between the experimental group and the two control groups presented no statistical significance(P0.05).The expressions of p53 and p21 in HCC tissue were weakly and negatively correlated,which had no statistical significance.The correlations among the expression rates of p53 and p21 in experimental group,control group A and B had no statistical significance.Both p53 and P21 were negative in all of the normal liver tissues.Conclusions During the process of hepatocarcinogenesis,p53 has a high expression rate and multifunction on promoting or inhibiting P21 expression,which might be caused by the synergistic effect of HBV and aflatoxin.
