摘  要:目的通过Meta分析的方法探讨PET或PET/CTSUV值对头颈部鳞癌预后的影响。方法检索PubMed、EM-BASE、Cochrane图书馆、CBM、CNKI和重庆维普等数据库,收集有关PET或PET/CTSUV值对头颈部鳞癌预后影响的病例进行对照研究,对纳入的研究进行质量评价,提取有效数据,采用RevMan4.2.8软件对总生存率(OS)、无病生存率(DFS)和局部控制率(LC)等进行Meta分析。结果共纳15个研究,包括461例患者。分析结果显示:高SUV值组和低SUV值组比较,2、3、5年的总生存率RR值及95%CI分别为0.66(0.55~0.80)、0.65(0.55~0.76)、0.49(0.33~0.72);无病生存率RR值及95%CI分别为0.70(0.62~0.79)、0.59(0.52~0.67)、0.65(0.49~0.86);局部控制率RR值及95%CI分别为0.76(0.66~0.88)、0.75(0.67~0.84)、0.52(0.33~0.85),差异均具有统计学意义(P<0.05)。结论 SUV值可能是头颈部鳞癌的独立预后因素,但须进一步开展多中心前瞻性随机对照研究,以确定SUV值的预后相关阈值及其临床指导意义。Objective To estimate the effect of PET or PET/CT SUV on prognosis of head and neck squamous cell carcinoma(NHSCC).Methods Cochrane systematic evaluation was used to search the databases of PUBMED,EMBASE,Cochrane Library controlled clinical trials,Chongqing VIP and Tsing Tongfang,supplemented with manual retrieval.After evaluating the quality of methodology and extracting the valid data,statistical analysis was performed with RevMan4.2.8 on overall survival rate,disease-free survival rate and local control rate.Results Fifteen clinical trials which included 461 patients from nine countries or regions were studied,of which thirteen trials were with high quality.The results of Meta-analysis showed that the values of OS,DFS and LC in high SUV group were significantly poorer than those in low SUV group.Conclusions Present evidences show that the SUV is a prognostic factor in patients with NHSCC.It is necessary to carry out multi-center prospective randomized controlled study for further confirming.
