食管癌术后吻合口瘘的诊治

摘　　要：目的讨论食管癌术后发生吻合口瘘的原因诊治及预防,以减少并发症的发生。方法对于405例食管癌切除术术后食管胃吻合重建,术后观察吻合口瘘的情况。结果 405例食管癌发生吻合口瘘23例,发生率为5.6%。结论吻合口瘘发生原因很复杂,与手术操作、术式、残胃与食管的血运、吻合口张力、胸腔感染及全身营养支持等因素有关。一旦发生,需采取有效的措施,可减少病死率。Objective To discuss the reason,diagnose and prevention of the anastomotic leakage after the operation of esophageal cancer,reducing the occurrence of the complication.Methods 405 cases were given esophago-gastroplasty after the operation of esophageal cancer.The incidence of anastomotic leakage was observed.Results Anastomotic leakage was happened to 23 examples in 405 examples,the occurrence rate was 5.65%.Conclusion The reason of anastomotic leakage is comprehensive,related to surgical operation,type of operation,blood circulation of esophageal and residual stomach,tension of anastomic stoma,thoracic cavity infections and nutritional support.Once happen,you can reduce the death rate by effective measure.
