摘 要:目的:研究正畸治疗中不同减数拔牙模式对牙颌面在垂直方向是否产生不同影响。方法:研究对象包括43名正畸患者,根据正畸治疗中的减数拔牙模式分为两组。A组为减数拔除4个第一双尖牙模式,包括26例患者,年龄15.1~19.6岁。B组为减数拔除4个第二双尖牙模式,包括17例患者,年龄15.2~17.8岁。全部患者遵循MBT矫治原则完成治疗,治疗中未使用头帽、Nance弓等加强支抗装置。每位患者治疗前后分别拍摄头颅定位侧位片,进行头影测量分析。使用配对t检验分别比较各组内减数拔牙正畸治疗后牙颌面的垂直方向变化,使用独立样本t检验比较两组间矫治前后牙颌面的垂直方向变化。结果:矫治开始前,两组患者在垂直方向的测量未发现显著性差异。正畸治疗后,减数拔除第一双尖牙组和减数拔除第二双尖牙组垂直方向无论骨骼测量还是牙齿测量均没有出现显著性差异(P>0.05),两组之间牙颌面在垂直方向的变化也没有出现显著性差异(P>0.05)。结论:正畸治疗中,不同的减数拔牙模式没有对颅面部骨骼和牙齿在垂直方向产生不同影响,因此,对于垂直发育过度患者,不宜依赖减数拔除牙弓后端牙齿达到垂直想控制的目的。Objective To study different influences of orthodonic treatments in different reduction extraction model for craniofacial vertical dimension.Methods There were 43 orthodonic patients who were divided into two groups according to different extraction model.Group A included 26 patients aged from 15.1 to 19.6,extraction four teeth of first premolars was the first step of orthodontic plan.Group B consisted of 17 patients aged from 15.2 to 17.8 and their four second premolars extracted for orthodontic treatment.No anchorage reinforcement was introduced.Treatment procedure followed MBT technique principles strictly.Cephalograms were taken on each patient before and after orthodontic treatment respectively.Cephalometrics was performed to evaluate the vertical dimension regarding to the skeletal and the dental.Independent-samples test was used to evaluate vertical dimension between group A and B before treatment.One-sample test was employed to compare the measurements before and after orthodontic treatment within either group A or B.Results Initially,there was no significant difference between group A and B(P0.05) in vertical plane of space.After orthodontic treatment,there was no significant skeleton difference was found in each group by comparing items before and after(P0.05).According to dental position,little differences emerged and did not get to the significant level(P0.05).Conclusion During orthodontic treatment,different extraction patterns had no effects on craniofacial vertical dimension both in terms of the skeletal and the dental.Therefore during orthodontic treatment planning,there was inappropriate to rely on posterior teeth extraction to control vertical dimension in vertical excess cases.
