摘 要:目的:探讨HIF-1α和E-cad在肺癌的发生、发展、浸润、转移过程中的作用及相互关系。方法:75例肺癌组织及其相应的41例癌旁组织,其中鳞状细胞癌44例、腺癌31例;高分化组17例、中分化组33例、低分化组25例;有淋巴结转移组52例、无淋巴结转移组23例。应用免疫组织化学方法检测HIF-1α蛋白和E-cad在肺癌及其癌旁组织中的表达。结果:①HIF-1α蛋白在肺癌组织(78.8%)中的表达显著高于癌旁组织(24.4%)(p<0.01);并随癌组织分化程度的降低呈逐渐增高趋势(p<0.01);有淋巴结转移组(94.2%)显著高于无淋巴结转移组(43.5%)(p<0.01);而与组织学类型无关。②E-cad在肺癌组织(24%)中的表达显著低于癌旁组织(87.8%)(p<0.01);并随癌组织分化程度的降低呈逐渐降低趋势(p<0.01);有淋巴结转移组(11.5%)显著低于无淋巴结转移组(52.2%)(p<0.01);而与组织学类型无关;③HIF-1α蛋白与E-cad在肺癌组织中的表达呈负相关(p<0.01)。结论:①HIF-1α是肺癌的早期事件,检测HIF-1α蛋白在肺癌中的表达水平,可作为判断肺癌恶性程度的一个重要参考指标。②E-cad表达异常参与肺癌浸润与转移。③低氧条件下HIF-1α可通过对其下游因子E-cad表达水平的调控作用,参与肺癌的侵袭、转移,这在肺癌发生发展过程中可能起重要作用,两者联合检测可以更准确地判断肺癌的恶性程度和生物学行为。OBJECTIVE:To detect the function of HIF-1αprotein,E-cadherin and the correlation of them in carcinogenesis,development,invasion and metastasis of lung cancer.METHOD:75 cases of lung carcinoma and 41 cases corresponding latero-carcinoma lung tissue.Among them,44 were squamous cell carcinomas,31 were adencarcinomas.17 were well-differentiated,33 were moderately differentiated,25 were poorly differentiated.52 examples of the lymph node metastasis appeared,23 were non-lymph node metastasis.Using Streptomycete Avidin-Biotin Complex(SABC) immunohistoche-mistry to examine the expression of HIF-1α protein and E-cadherin in 75 cases lung carcinoma.RESULTS:①The positive rate of HIF-1α protein in lung carcinoma(78.7%) was significantly higher than those in spherical-carcinoma(24.4%)(p0.01),and was higher gradually with the lower different differentiation carcinoma(p0.01),there is a significant difference between the lymph node metastasis group(94.2%) and non-lymph node metastasis group(43.5%)(p0.01),while there is no significant difference between different histology types(p0.05).②The positive rate of E-cadherin in lung carcinoma(24%),was significantly higher than those in spherical-carcinoma(87.8%)(p 0.01),and was lower gradually with the higher different differentiation carcinoma tissues(p0.01),There is a significant difference between the lymph node metastasis group(11.5%) and non-lymph node metastasis group(52.2%)(p0.01),while there is no significant difference between different histology types(p0.05).③There was a negative correlation between the positive expression rate of HIF-1αprotein and that of E-cadherin in lung carcinoma(p0.01).CONCLUSION:①The expression of HIF-1α is early thing in lung carcinoma,which showed that detection of HIF-1α expression could be a good factor for monitoring the severity and prognosis of lung carcinoma.②E-cadherin might be a reliable factor in evaluating the biological behavior of lung carcinoma
