摘  要:目的:比较自制的超顺磁性氧化铁(superparamagnetic iron oxide,SPIO)和二乙烯五胺乙酸钆(Gd-DT-PA)用于磁共振诊断肝癌的效果。方法:大耳白兔24只,随机分为两组,每组12只,开腹直视接种法建立VX2肝癌模型。种植后7天、14天分别进行MR平扫及增强扫描并与病理组织学结合评价诊断正确率。结果:种植后7天12个病灶SPIO确诊10个(83.3%),Gd-DTPA13个病灶确诊为9个(69.2%),两组比较差异有统计学意义(P<0.05);种植后14天19个病灶SPIO确诊16个(84.2%),Gd-DTPA17个病灶确诊15个(88.2%),差异无统计学意义(P>0.05)。结论:合成的SPIO可作为MR造影剂诊断早期小病灶肝癌。Objective:To compare the difference between SPIO and Gd-DTPA in diagnosis of hepatic carcinoma by MR scan after intravenous injection of SPIO and Gd-DTPA respectively.Methods:A total of 24 rabbits were randomly divided into two equal groups to construct VX2 hepatic carcinoma models via the laparotomic route method.MR scan was performed after tissue suspension was implanted into hepar on 7thd and 14 thd and pathologic histology of tumor was performed.Results:In the diagnosis of hepatic tumor of the early stage,the accurate ratio of the SPIO group was 83.3%,while that of the Gd-DTPA group was 69.2%,with statistical difference.At the late stage,there was no statistical difference between the two groups.Conclusion:It was more accurate to diagnose early small hepatic carcinoma by intravenous injection of SPIO than by Gd-DTPA,while there was no difference in the diagnosis of moderate and late large ones.
