摘 要:目的探讨急性缺血性脑梗死患者颅内脑动脉狭窄或闭塞后侧枝循环形成情况。方法 235例经影像学发现存在颅内动脉狭窄或闭塞的急性缺血性脑梗死患者,依据有无侧支循环分为两组,对可能与侧支循环建立有关的影响因素进行单因素及logistic多因素回归分析。结果 (1)侧支循环形成的检出率DSA最高,MRA次之;(2)Logistic回归分析显示:以侧支循环开放作为因变量时大脑中动脉狭窄、颈内动脉闭塞、基底动脉狭窄进入回归方程,OR分别为0.598、2.395、3.494;P值分别为0.104、0.081、0.015。结论大脑中动脉狭窄、颈内动脉闭塞、基底动脉狭窄可能促进侧支循环开放。Objective To explore the cerebral collateral circulation establishment situation in acute infarction patients with intracranial artery stenosis or occlusion.Methods 235 patients with cerebral infarction,whose intracranial arterial stenosis or occlusion were located by imaging,were divided into two groups according to whether the collateral circulation or not,and the promoting factors for establishment of collateral circulation were analyzed by univariate analysis and multinomial logistic regression analysis.Results Univariate analysis showed the highest detection rate of methods for collateral circulation was DSA,and MRA was lower.Logistic regression analysis showed that with collateral circulation as the dependent variable,middle cerebral artery(MCA) stenosis,internal carotid artery(ICA) occlusion and Basilar artery(BA) stenosis were entered into the equation,and OR were 0.598,2.395 and 3.494,with P values were 0.104,0.081,0.015.Conclusion Middle cerebral artery(MCA) stenosis,internal carotid artery(ICA) occlusion and basilar artery(BA) stenosis may stimulate cerebral collateral circulation establishment.
