口腔颌面部骨折39例临床探讨

摘　　要：目的探讨口腔颌面部骨折及并发症骨折的诊治要点和注意的问题。方法回顾性分析39例口腔颌面部骨折并发全身性其它损伤病例。有严重并发症患者,要首先抢救生命,及早治疗颌面部骨折,处理骨折行清创,手术复位,内固定,恢复面部外形及咬颌关系,分别采用单颌固定,颌间牵引固定,微型钛板坚固内固定。结果本组38例口腔颌面部患者咬合关系恢复正常,骨折愈合良好,张口度、面部外形基本正常。1例复杂骨折入院时因伴脑挫伤昏迷,伤后3周病情稳定,骨折己错位愈合,张口度、咬合关系、面部外形均有重度障碍。结论口腔颌面部骨折处理的原则是首先抢救生命,在此基础上早期复位、内固定,恢复面部外形及咬颌关系。Objective To evaluate the oral and maxillofacial diagnosis and treatment of fractures and fracture complications. Methods A retrospective analysis of 39 cases of oral and maxillofa- cial fracture and other damage of systemic cases. Patients with serious complications, should be the first to save lives, and early treatment of maxillofacial fractures, fracture debridement, open reduction, internal fixation to restore relations between facial appearance and occlusion were single jaw fixed, fixed traction, micro -titanium plate fixation. Results 38 cases of oral and maxillofacial patients return to normal occlusion, fracture healing well, mouth opening, facial appearance was normal. 1 case of complex fractures admitted to hospital with cerebral contusion due to coma, stable disease 3 weeks after injury, fracture healing has been misplaced, mouth opening, occlusion, facial appearance are se- vere obstacles. Conclusion The treatment of oral and maxillofaeial fractures is the first principle of saving lives, on the basis of early reduction, internal fixation to restore facial appearance and occlusion relations.
