BSD热疗联合放化疗同步治疗腹膜后淋巴结转移癌临床观察

摘　　要：目的探讨BSD热疗联合放化疗同步治疗食管癌及贲门癌腹膜后淋巴结转移的近期疗效及安全性。方法 46例食管癌及贲门癌腹膜后淋巴结转移随机分为两组。治疗组采用三维适形放疗(3D-CRT)及氟脲嘧啶持续泵入化疗配合BSD热疗。3D-CRT分割剂量2～3 Gy/次,5次/周,总剂量45～60 Gy/15～30次,3～6周。氟脲嘧啶0.25 g/(m2.d),持续静脉注射,连用10 d。BSD热疗每周2次,直至治疗结束。对照组仅给予放化疗,方法同治疗组。治疗后两组分别评价疗效,同时检测两组治疗前后T细胞亚群变化。结果治疗组总有效率为70.8%,症状缓解率为85.7%;对照组总有效率为54.5%,症状缓解率为63.2%;差异均有统计学意义(P<0.05)。治疗后治疗组T细胞亚群中CD3+、CD4+、CD4+/CD8+均较治疗前有一定程度升高,且亦较对照组治疗后升高(P<0.05)。两组Ⅱ～Ⅲ度骨髓抑制发生率差异无统计学意义(P>0.05)。结论 BSD热疗联合放化疗同步治疗食管癌及贲门癌腹膜后淋巴结转移是安全有效的。Objective To investigate the efficacy and toxicities of BSD thermotherapy plus radiotherapy and chemotherapy retroperitoneal lymph node metastases of esophageal and gastric cardia carcinoma.Methods Forty six patients were divided into two groups randomly.The treatment group will be treated by BSD thermotherapy plus three dimensional conformal radiation therapy（3D-CRT） and fluorouracil chemotherapy.3D-CRT： Total dose was 45-60 Gy in 3-6 weeks,2-3 Gy/times,five times per week.Fluorouracil 0.25 g/（m2？？d）,civ,d1-10.At the same time,received BSD thermotherapy twice per week.The control group received 3D-CRT and fluorouracil chemotherapy.Results In the treatment group,the overall response rate was 70.8%,and the remission rate of symptoms was 85.7%;while in the control group,the overall response rate was 54.5%,and the remission rate of symptoms was 63.2%.The overall response rate and remission rate of symptoms in the treatment group was higher than those of the control group（P0.05）.The CD3＋,CD4＋, CD4＋/CD8＋T lymphocyte in the treatment group were more than those of the control group（P0.05）.No significant difference was found in ？？？ grade marrow depression between the two groups.Conclusion Radiotherapy and chemotherapy combined with BSD thermotherapy is effective and safe for retroperitoneal lymph node metastases of esophageal and gastric cardia carcinoma.
