摘  要:目的探讨自体角膜缘干细胞移植治疗翼状胬肉手术中应用丝裂霉素C对角膜内皮、角膜上皮的影响。方法本实验组筛选46例原发性翼状胬肉的患者(46只眼),所有患者均接受胬肉切除手术,然后行角膜缘干细胞移植联合丝裂霉素C。分别测量术前、术后角膜内皮细胞密度、变异系数及六角形细胞百分比,观察角膜创面上皮愈合情况并记录愈合时间。结果随访期内胬肉复发1例(2.2%)。各阶段角膜内皮细胞密度、平均变异系数以及平均六角形细胞百分比值比较无明显差异(P>0.05)。角膜荧光素染色阴性平均时间为(4.20±1.05)d。结论翼状胬肉切除术联合自体角膜缘干细胞移植术中使用0.02%丝裂霉素C延长术后角膜创面愈合时间,但不会对角膜内皮产生急性毒性作用,手术复发率低。Objective To investigate effects of corneal limbal stem cell transplation and mitomycin C on the basis of the simple pteryium excision on corneal epithelium and endothelium. Methods 46 patients (46 eyes) with primary pterygium were included in the study. All patients underwent corneal limbal stem cell transplation and mitomycin C on the basis of the simple pteryium excision. To determine cell density, percentage of hexagonal cells, and coefficient of variation(CV) of cell size before and after surgery. Wound healing of corneal epithelium was observed and healing time recorded. Results During the follow-up period, one eye was found to have recurrent pterygium (2.2%). There were no significant differences among cell density, percentage of hexagonal cells, and coefficient of variation(CV) of cell size in all stages(P0.05). Average time of corneal fluorescence staining with negative expression was (4.20±1.05)d. Conclusion 0.02% MMC prolonged healing time of corneal wound of patients undergo corneal limbal stem cell transplation on the basis of the simple pteryium excision. No acute toxicity on corneal epithelium is observed and the recurrent rates are relatively lower.
