肺癌血液循环的数字图像动态观察

摘　　要：目的　应用数字图像系统，动态观察肺癌血液循环。方法　经股动、静脉插管对１４７ 例肺癌行２２１ 次支气管动脉造影和化疗灌注，１３ 例同时作肺动脉造影。均以ＤＳＡ 数字成像系统或电影取像１２５ ～２５ 帧／ｓ。结果　①原发性肺癌主要为单支体循环动脉供血：支气管动脉占８４ ％ ，１３ 例肺动脉造影均不向肺癌供血。②介入治疗过程中，肺癌供血动脉可发生改变：本组３ 例，原由支气管动脉或肋间动脉供血，经介入治疗几次后而改由其它体循环动脉供血。③转移性肺癌多由支气管动脉供血，一例为肺静脉供血。结论　肺癌的供血情况以动态数字图像显示最为满意，肿瘤染色，肺动静脉瘘等尤其是用ＤＳＡ 为明确。肺癌供血除支气管动脉外，不少由其他体循环动脉供血，甚至肺静脉供血。在介入治疗期间，原供血动脉可以改变，要注意寻找。Purpose Observed the blood circulation of pulmonary carcinoma dynamicly by digital imaging technique.Methods Interventional therapy of 147 cases of pulmonary cancer in which 221 angiographies and chemicals infusions were performed.Pulmonary arteriographies of 13 cases had been made also.Angiographic images were collected by digital image system(DSA and movie,12.5 frames/sec.)Results ①Primary pulmonary carcinoma were feed by the systemic arteries,mainly by bronchial arteries(84%).There were no blood supply founded by 13 pulmonary arteriographies.② 3 cases of primary lung cancer supplied originally by bronchial arteries which obliterated after first therapy and substituted by other systemic arteries.③Matastatic pulmonary carcinoma were supplied by bronchial arteries generally and 1 case of them supplied by pulmonary vein.Conclusion Dynamic observation of digital imaging technique especially DSA is best in use of manifestation of blood circulation of lung cancer.Pulmonary cancer may be supplied by not only one systemic artery but even pulmonary vein,and the feeding artery may be changeable during the period of the interventional therapy.
