胃肿瘤中增殖细胞核抗原表达的临床研究

摘　　要：为了解增殖细胞核抗原（ＰＣＮＡ）在胃肿瘤中的表达及其临床意义，应用免疫组化技术（ＡＢＣ法），对胃肿瘤（胃腺瘤性息肉１０例，胃癌９４例）组织切片中ＰＣＮＡ含量进行半定量检测。结果：①胃粘膜不同病理状态下ＰＣＮＡ指数差异有极显著性意义（Ｐ＜０．０１）；②ＰＣＮＡ指数与患者的性别和年龄无关（Ｐ＞０．０５），而与肿瘤的生长方式、组织类型、浆膜层受侵与否、淋巴结转移与否和临床分期关系密切（Ｐ＜０．０５）；③ＰＣＮＡ指数与胃癌的预后密切相关（Ｐ＜０．０５），弱级（Ⅰ～Ⅱ级）表达的患者预后好。由此表明，ＰＣＮＡ检测可作为判断胃癌生物学行为和预后的良好指标之一。To study the expression of proliferating cell nuclear antigen (PCNA) in human gastric neoplasm and its clinical significance. The expression of PCNA was semiquantitatively analysed immunohistochemically (ABC) in 10 gastric adenomatous polyp and 94 gastric carcinomas. Results: PCNA labeling index (LI) showed a significant differentiation in different pathologic state (P<0.01), PCNA LI in gastric carcinoma was indepednet of sex and age (P>0.05), but correlated with the growing manner of the tumor, tumor differentiation, serosal infiltration, nodal metastasis and clinical stages (P<0.05), The correlation was found between PCNA LI and prognosis of gastric carcinoma (P<0.05). A moderate grade of PCNA expression generally had a better prognosis. Conclusions: These results suggest that PCNA LI is a fairly good index to indicate biologic behavior and prognosis in gastric carcinoma.
