M-VCA方案治疗晚期鼻咽癌35例疗效分析

摘　　要：目的：探讨ＭＶＣＡ方案治疗晚期鼻咽癌的疗效及毒副反应。方法：对３５例晚期鼻咽癌患者（初治诱导化疗者１１例，复发或转移者２４例）采用ＭＴＸ＋ＶＣＲ＋ＤＤＰ＋ＡＤＭ联合化疗方案治疗。结果：评定其化疗２疗程后的客观疗效：总有效率为７５５１％，无ＣＲ病例，其中初治肿瘤有效率达１００％，复发者为６６６７％，远处转移总有效率为７００％，肺转移疗效最好（８８８９％）。主要毒副反应是脱发，轻到中度的骨髓抑制和胃肠道反应。结论：ＭＶＣＡ方案治疗晚期鼻咽癌，无论初治或再治，都具有效率较高，使用方便，副作用可耐受的优点，可在临床进一步观察使用。Objective: To observe and evaluate the efficacy and toxicity of MVCA regimen on advanced nasopharyngeal carcinoma. Methods: 35 patient with advanced naspoharyngeal carcinoma, including 11 cases of original localregional advanced nasopharyngeal carcinoma and 24 cases, localregional recurrent or/and metastatic nasopharyngeal carcinoma,were treated with MVCA(methotrexate:30 mg/m2 d1,vincristine:2mg d1,cisplatin:70 mg/m2 d1,doxorubicin 30 mg/m2 d1) regimen.Cycles were repeated every 3 weeks and 2 cycles were delivered. Results: The overall response rate was 7551%the original cases:100%,the recurrent cases:666%and the overall response rate of metastatic cases:700%,respectively.The main toxicities included mild to moderate degree of alopecia,nausea/vomiting and neutropenia. Conclusion: The results of this study demonstrate thet MVCA regime has good tolerability and efficacy in original localregional cavanced,recurrent and/or metastatic nasopharyngeal carcinoma, and further investigation is worthwhile.
