摘 要:目的探讨主穹窿蛋白(major vault protein MVP)在口咽鳞癌中的表达及其与口咽鳞癌临床病理特征和预后的关系。方法回顾分析73例口咽鳞癌临床病理资料,免疫组化法检测MVP表达,分析MVP表达与口咽鳞癌临床病理及预后的相关性。结果 73例MVP表达阳性率为67.1%,MVP表达阳性患者颈部淋巴结转移率较MVP表达阴性患者高,对放、化疗的敏感性下降,差异均有统计学意义(均P<0.05)。5年总生存率为24.2%,MVP表达阳性和阴性患者5年生存率分别为6.0%和51.7%,差异有统计学意义(P<0.001)。MVP表达与口咽癌病理分化和T分期无相关性,多因素分析显示MVP表达是口咽鳞癌的独立预后因素(P<0.001)。结论 MVP在口咽鳞癌中表达水平较高,MVP的高表达增加口咽鳞癌患者颈部淋巴结转移风险,可以作为口咽鳞癌患者放、化疗敏感性和预后的预测指标。Objective To investigate the expression of MVP in oropharyngeal squamous cell carcinoma (OSCC) tissues and analysis its relation to clinical and pathologic prognostic factors and its role in predicting clinical outcome of OSCC. Methods Analysis respectively the record of clinical and pathologic prognostic fac- tors of 73 consecutive patients suffering from oropharyngeal cancer, and assess the relation between clinical and pathologic prognostic factors and MVP expression studied by immunohistochemistry. Results Positive staining for MVP was found in 64.2% OSCC, the positive rate of MVP in OSCC patients with lymph node involvement was higher than that without lymph node metastasis, and had a significant difference ( P ( 0.05 ) , OSCC pa- tients with high MVP expression was resistant to chemotherapy or radiotherapy, there was a significant difference (P 〈0.05). The total 5-year survival rate was 24.2% in the cohort, and the 5-year survival rate was 51.7% in patients with negative MVP expression and 6.0% in the positive respectively, There was a significant difference (P 〈0.01 ). No significant correlation was observed between MVP expression and cell differentiation and T staging. Multivariate analysis indicated MVP remained a highly significant independent prognostic factor for clin- ical outcome ( P 〈 O. 001 ). Conclusions The expression level of MVP was higher in OSCC, MVP expression enhance the hazard of lymph node metastasis, and can be regarded as an independent marker to predict sensitivity of chemotherapy or radiotherapy and prognosis of OSCC.
