宫颈癌体外照射真空袋体位固定技术的分析

摘　　要：目的通过对宫颈癌体外照射真空袋组和非真空袋组两种体位固定技术摆位误差的比较,分析真空袋体位固定技术的临床应用价值,以提高放疗摆位精度和治疗效果。方法 50例宫颈癌体外照射患者随机分为真空袋组和非真空袋组各25例,第1周～第4周周末在加速器下摄验证片,与定位片比较误差大小。结果真空袋组与非真空袋组三个方向的摆位误差比较:头脚方向两组差异无统计学意义(t=1.85,P>0.05),说明头脚方向真空袋组未能减少摆位误差;左右方向和前后方向差异均有统计学意义(t=8.63,P<0.01;t=11.89,P<0.01),说明真空袋组比非真空袋组在左右、前后方向摆位误差明显减少。结论使用真空袋可减少宫颈癌体外照射的摆位误差,提高放疗摆位精度和治疗效果,且摆位步骤简便易行,具有临床推广价值。Objective By comparing the set-up uncertainty of two technique of immobilization methods in external radiotherapy of cervical cancer, to analyze the clinical value of technique of immobilization about the vacuum bag for improving set-up precision and treatment effect of radiotherapy. Methods 50 cervical cancer patients were randomly divided into vacuum hag group and non-vacuum bag group, each group had 25 cases. The film was taken at the weekends of first to fourth week with the treatment position in simulator for each patient, then these films were compared and set-up uncertainty was measured. Results The set-up uncertainty of three directions in vacuum bag group and non-vacuum bag group was compared： there was no significant difference in cranio-caudal （C-C） direction of the two groups （t = 1.85, P 〉0.05）, which explained the C-C direction of vacuum bag group did not reduce the set-up uncertainty; there was significant difference in medio-lateral （M-L） and antero-posterior （A-P） directions of two groups （t=-8.63, P〈0.01; t=l 1.89, P〈0.01）, which explained in M-L and A-P directions the set-up uncertainty was significantly lower in vacuum bag group than that in non-vacuum bag group. Conclusion Us- ing vacuum bag can reduce the set-up uncertainty in external radiotherapy of cervical cancer and improve the set-up precision and treatment effect of radiotherapy, the set-up procedure is simple and easy, which has the value of clinical practice.
