细胞色素P450酶1A2 mRNA表达水平与氯氮平浓度及疗效关联性研究

摘　　要：目的探讨精神分裂症患者外周血单核细胞中CYP1A2mRNA表达水平与氯氮平疗效及血药浓度的关系。方法应用RT-PCR技术测定60例精神分裂症患者外周血单核细胞CYP1A2mRNA表达水平,临床症状和疗效的评价使用阳性和阴性症状量表(PANSS)和简明精神病量表(BPRS);氯氮平血药浓度监测采用反相高效液相色谱法测定。结果在实验第4周末、第8周末的氯氮平的血药浓度与CYP1A2 mRNA表达水平呈负相关(r分别为-0.389,-0.487,P<0.05)。第8周的阳性和阴性症状量表PANSS减分率和简明精神病量表BPRS减分率与CYP1A2 mRNA表达水平不存在相关关系。结论精神分裂症患者CYP1A2的mRNA表达水平与氯氮平的疗效无关联,与氯氮平的血药浓度呈负相关。Objective To explore the relationslip between cytoehrome CYP1A2 mRNA expression in peripheral blood mononuelear cells in schizophrenic patients and the serum concentration of elozapine, as well as the efficacy of elozapine. Methods The mRNA expression levels of eytoehrome P450 1 A2 in the peripheral blood mononuelear cells in 60 schizophrenic patients were measured with real - time polymerase chain reaction technique. The clinical symptom and efficacy were evaluated by using the positive and negative syndrome scale （PANSS） and the brief psychiatric rating scale （BPRS）. Serum elozapine concentration were measured with reversed phase high -performance liquid chromatography （ RP - HPLC）. Results There were significant negative correlation between serum elozapine concentration at the end of 4^th week,8^th week and the mRNA expression levels of CYP1A2. （r = -0. 389, -0. 487, P 〈 0.05）. There was no significant correlation between PANSS, BPRS reduction rate at the end of 4^th week,8^th week and the mRNA expression levels of CYP1A2. Conclusion There is no correlation between the mRNA expression level of CYP1A2 and efficacy of elozapine. There is a nagative correlation between CYP1A2 mRNA expression and serum elozapine concentration in schizophrenic patients.
