摘  要:目的:通过分析多种白介素等细胞因子与纤维蛋白原(Fibrinogen,Fg)Bβ链-854G/A、-455G/A、-249C/T、-148C/T、448G/A和Bcl-1G/A六个具有代表性位点基因多态性的关系及其对血浆Fg浓度和分子活性系列指标的影响,探讨白介素等细胞因子与血浆Fg浓度和分子活性基因表达的相关性。方法:采用整群抽样的方法选取开滦集团离退休职工1 391人进行体检和问卷调查,抽取静脉血测定血生化及血C反应蛋白(CRP)、肿瘤坏死因子(TNFα-)和白介素6(IL-6)、IL-8、IL-10,同时测定血浆Fg浓度和纤维蛋白单体聚合反应速率(FMPV)、最大光密度(Amax)、FMPV/Amax等反映Fg分子聚合功能指标,同时应用聚合酶链反应-限制性内切酶片段长度多态性技术进行六位点的基因多态性检测。结果:FgBβ-854位点各基因型人群间IL-6、FM-PV/Amax及Fg浓度差异均有显著性意义(P<0.05);Bcl-1各基因型人群间IL-8有差异性(P<0.05)。多元逐步回归分析,以IL-6和IL-8为因变量时筛选出Bβ-854、-249、Bcl-1、Fg浓度(P<0.05);以IL-10和TNFα-为因变量时则筛选出Bβ448、Bcl-1、FM-PV/Amax(P<0.05);以FMPV/Amax为因变量,依次筛选出Bβ448AA、Bβ-854GA、Fg浓度、IL-10、TNFα-(P<0.05)。结论:血浆Fg浓度和分子聚合功能的基因调控和表达与多种因素和多部位基因位点可能有关,其中血浆Fg浓度与IL-6、IL-8等因子和FgBβ-854、-249及Bcl-1等基因位点可能有关,而Fg分子聚合活性则与TNFα-、IL-10等细胞因子和FgBβ448、Bcl-1等基因位点可能相关联,同时血浆Fg浓度与其分子聚合活性的表达可能交互影响。Objective:To study the linkage of cytokines and Bβ-854G/A,-455G/A,-249C/T,-148C/T,448G/A,Bcl-1G/A gene polymorphisms with fibrinogen functional features such as plasma fibrinogen concentration and molecular reactivity.Methods:1 391 subjects of retired employees from Kailuan Group were enrolled in the medical examination and questionnaire survey.C reactive protein(CRP),tumor necrosis factor(TNF-α),interleukin-6(IL-6),IL-8,IL-10 and blood biochemistry index,Fg concentration,fibrin monomer polymerized velocity(FMPV),absorbance maximum(Amax) and FMPV/Amax were measured in all the individuals.The gene polymorphisms at six sites were detected by polymerase chain reaction-restriction fragment length polymorphism.Results:IL-6,FMPV/Amax and Fg concentration were significantly different among Bβ-854 genotypes(P〈0.05).IL-8 was different among BβBcl-1 genotypes(P〈0.05).The result of multiple stepwise regression analysis showed that IL-6 and IL-8 as dependent variable.Fg concentration and locus gene polymorphisms on Bβ-854,-249,Bcl-1 were selected(P〈0.05);With IL-10 and TNF-α as dependent variable,FMPV/Amax and locus genes on Bβ448,Bcl-1 were entry(P〈0.05);FMPV/Amax as dependent variable,Bβ448AA genotype,Bβ-854GA genotype,Fg concentration,IL-10 and TNF-α were selected in turn(P〈0.05).Conclusion:Gene regulation,plasma fibrinogen concentration and molecular reactivity in body have correlation with multiple factors and multiple locus gene polymorphisms on Fg beta-chain.It is found that the plasma fibrinogen concentration was significantly associated with the factors of IL-6,IL-8,and locus gene polymorphisms on FgBβ-854,-249 and Bcl-1.The associations of plasma fibrinogen molecular reactivity in body with the cytokine of TNF-α and IL-10,locus gene on FgBβ448 and Bcl-1 were significant on our results.Plasma fibrinogen concentration and molecular reactivity in body can be regulated each other.
